Purpose: Through 5 years prospective clinical experiment,the aim of this investigation was to observe and evaluate the long-term clinical outcomes of a new implant system.Furthermore,the study wsa to explore the influences of sandblasted,large grid,and acid-etched(SLA)surface modification on osseointegration and to assess the influences of platform switching,Morse tapered and microthreaded neck designs on both hard and soft tissues,providing theoretical and practical guidance for the selection of implants in clinial work.Ultimately,it was to promote the application and popularization of the domestic implants.Methods: From Nov.2011 to Jan.2012,120 patients of single odontoptosis who needed implant restoration were treated by 120 sandblasted,large grid,and acid-etched(SLA)surface implants,who were enrolled from the Affiliated Hospital of Qingdao University and Qingdao Municipal Hospital.All of the implants were rehabilitated with single metal–ceramic crowns and were cemented to a titanium abutment individualized by the dental technician in two to four months after surgery.The patients were recalled at follow-up visits scheduled postoperation,prosthesis installation,1-year and 5-year after surgery for clinial examinations,including plaque index(PI)to assess oral hygiene levels,gingival index(GI),pocket probing depth(PPD),the width of the keratinized mucosa to assess the soft tissue health around implants,checking for implant/prosthesis related complications and evaluating the osseointegration and marginal bone remodeling through radiograhic examination.Results:1.Favorable osseointegration was formed after 2-4 months for 120 implants,and the accumulative survival rate of implants was 100% with no implants loosening and loss.2.After 5 years follow-up,the soft tissue around implants was stable,with no peri-implantitis.Apart from 4 porcelain fractures,there were no implant/prosthesis mechanical complications.3.According to the Albrektsson’s criteria,the success rates of implants at the 1-year and the 5-year were 100% and 99.1%,respectively.4.The mesial marginal bone height at postoperation,prosthesis installation,1-year and5-year after surgry is(0.29±0.22)mm,(-0.03±0.46)mm,(-0.09±0.34)mm,(-0.12±0.28)mm respectively.The distal marginal bone height is(0.28 ± 0.22)mm,(-0.04 ±0.35)mm,(-0.11±0.33)mm,(-0.15±0.33)mm respectively.The mesial marginal bone loss at 1 year and 5 years after surgry were(-0.38±0.32)mm,(-0.42±0.33)mm.5.At mesial,the main change were found before the prosthesis insertion.At distal,the main change were found 1 year after surgery.There was no significant difference between the 1-year and the 5-year bone level(P>0.05).Conclusion:1.The sandblasted,large grid,and acid-etched(SLA)surface treatment can promote the osseointegration,and make the repair time short.planting system could be considered as a manageable and implant system.2.Platform switching,Morse tapered connection and microtheaded neck design can decrease the marginal bone losss effectively,maintaining the health of the soft tissue and are advantageous to the long-term stability of the implant.3.There is a long-term satisfactory result for the implants which have high survival and success rates of 5 years...
