| Objectives:To evaluate and compare the losing of marginal bone and the position changes of gingival after immediate implant and functional loading of dental implant with platform-switching implants and butt-joint implants at the short term, and investigate the success rate factors of immediate implant and functional loading. At the same time, to observe the position changes of gingival.Methods:Select50patients, whose upper and lower front teeth or first premolar was could not be reserved because of the root fracture due to trauma, dental defects or disease.Immediate implant and functional load after be pulled out. Randomly divided into two groups. One group use the immediate-loading of dental implant with platform-switching implants and another group use the immediate-loading of dental implant with butt-joint implants. After12months, the two group’s restorations are replaced by a permanent restoration. Imaging tests carried out in all cases (include CT, Panoramic film, Periapical films). Evaluate and compare the marginal bone’s losing of two kinds of implants by imaging detection and observe the position changes of gingival.Results:The implant success rate of two groups are100%, the short-term clinical effects of upper and lower front teeth (after be pullde out.) using immediate-loading of dental implant with platform-switching implants is good. The mean values of the marginal bone losing of platform-switching implants is lower than butt-joint implants. But it has no significant difference. The gingival position of the two groups:the probing depth after the operation was>3mm, and it returned to normal after3months in platform-switching implants. The average value of PD of platform-switching implants is2.68mm and another group is2.98mm, P<0.05and it has significant difference.Conclusions:The losing of marginal bone has no significant difference at the short term while immediate implant and functional loading using platform switching and butt-joint. But the gingival probing depth is better than the butt-joint. It has higher success rate and satisfactory clinical results.So it have a high clinical value. |
