Background:In recent years, studies abroad have shown that moderate physical activity in patients with colorectal cancer can improve their health outcomes. However, the current situation of physical activity in patients with colorectal cancer abroad is generally not good.Foreign scholars have made a preliminary exploration of the influencing factors, and have begun intervention research.Meanwhile, the current research on physical activity in China is mainly focused on the general population and the population with chronic diseases, and less attention has been paid to the tumor population. Here is still lack of the study on current situation of physical activity and its influencing factors in patients with colorectal cancer. Therefore, this study aims to explore the physical activity status of colorectal cancer patients and its influencing factors in order to provide reference and suggestions for targeted development of physical activity strategies for patients with colorectal cancer.Objective:(1)To describe the current status of physical activity in patients with colorectal cancer;(2)To explore the influencing factors of physical activity in patients with colorectal cancer. Methods:This study was based on Health Promotion Model.European Prospective Investigation into Cancer and Nutrition(EPIC)Physical Activity Questionnaire were translated into Chinese version, combined with the general information questionnaire,EORTC-QLQ-C30 and EORTC-QLQ-CR38 Chinese version, the Hospital Anxiety and Depression Scale,Exercise Benefits/Barriers Scale,Exercise Self-Efficacy Scale and Social Support Rating Scale as the investigation tools.Convenience sampling was used to investigate 168 colorectal cancer patients in China-Japan Friendship Hospital,Beijing Hospital and Civil Aviation General Hospital from April 2017 to October 2017.SPSS22.0 was used for statistical analysis, including descriptive analysis, correlation analysis, single factor analysis and multiple stepwise regression analysis.Results:(1)The test-retest reliability of Chinese version of the EPIC physical activity questionnaire was 0.818;the content validity index was 0.900;the International Physical Activity Questionnaire-Short Form as criterion, the criterion validity was 0.417;discriminant validity test showed that the physical activity score in patients with non-colorectal cancer patients was significantly higher than that in patients with colorectal cancer(Z=2.868,P=0.004)(2)The highest score of physical activity in colorectal cancer patients is 296.85,the lowest was 5.25,the median was 65.52,the lower and upper quartile were 38.35 and 98.00;the median time of Moderate-to-Vigorous Physical Activity(MVPA)in patients with colorectal cancer per week was 0.39 hours, the median time of mild to moderate physical activity weekly is 17.50 hours;only 25.60%of the patients with colorectal cancer survivors reached the requirement of Colorectal Cancer Survivorship Care Guidelines;according to work and leisure physical activity time the respondents can be divided into groups of four categories, including inactive group with 18 patients(10.71%),moderately Inactive group with 85 patients(50.60%),moderately active group with 62 patients(36.90%),active group with 3 patients(1.79%).(3)The results of single factor analysis of patients with colorectal cancer showed physical activity status in patients without job was better than those on-the-job(F=-2.169,P=0.030);the number of complications in patients with colorectal cancer, fatigue and depression were negatively correlated with physical activity status, the differences were statistically significant(P<0.05);body image of patients with colorectal cancer, perceived exercise benefits and barriers and self-efficacy of exercise were positively correlated with physical activity status, the differences were statistically significant(P<0.05).(4)Multiple stepwise regression analysis showed that the influencing factors of physical activity according to effect from large to small is as follows:perceived exercise benefits/barriers,fatigue,depression and employment status. These four variables explained 25.4%of the total variance of the maximum activity score. The higher perceived benefits of exercise, the lower perceived barriers, the better physical activity status;with more fatigue, patients' physical activity is worse;with more severe depression, patients' physical activity status is worse;physical activity in patients without job was better than those on-the-job.Conclusion:The Chinese version of European Prospective Investigation into Cancer and Nutrition(EPIC)Physical Activity Questionnaire was developed. This questionnaire has good reliability and validity and can be used to measure the physical activity of patients with colorectal cancer. The physical activity status of patients with colorectal cancer is not optimistic. The main influencing factors are perceived exercise benefits/barriers and depression. In addition, the correlate factors are number of comorbidities, body image and exercise self-efficacy.The factors that can be intervened are fatigue and depression, perceived exercise benefits/barriers and self-efficacy of exercise. It suggests medical staff should timely assess the colorectal cancer patients' physical activity, focus on patients on-the job and those with many complications or body image disorder, timely assess fatigue and depression of patients and provide scientific and individual physical activity guidelines for patients to improve the perceived exercise benefits/barriers and exercise self-efficacy and improve the physical activity status of patients with colorectal cancer in final.
