This article summarizes the clinical study of western medicine for the treatment of painful diabetic peripheral neuropathy and traditional Chinese medicine for the treatment of diabetic peripheral neuropathy respectively.The part of clinical research is a retrospective analysis of painful diabetic peripheral neuropathy treated by traditional Chinese medicine.A total of 30 cases were included in the study,whose visit information of 0,3 and 6 months was recorded,a total of 90 visits.OBJECTIVES:(1)To evaluate the improvement of NRS pain scores and main symptoms in patients with painful diabetic peripheral neuropathy;(2)To evaluate the influence of Chinese herbs on blood glucose;(3)Evaluate the safety of traditional Chinese medicine in the treatment of painful diabetic peripheral neuropathy;(4)Summarize the ideas and characteristics of Professor Xiaolin Tong for treatment of this disease;(5)Organize and analyze“simultaneous treatment of blood glucose and collaterals”academic thought of Professor Xiaolin Tong in Diabetic Microvascular Complications.METHODS:According to the inclusion and exclusion criteria,the cases were screened.The treatment time of 0,3 and 6 months are considered as the observation time points.The Case Record Tables were prepared and filled out.The electronic database was established through Excel2007.And use SPSS 20.0 to process data,draw icons,and finally do analysis.RESULTS:This study included 30 cases of moderate and severe painful diabetic peripheral neuropathy with NRS pain score?4 who went to Professor Xiaolin Tong' clinic of Guang'anmen Hospital China Academy of Chinese Medical Sciences for treatment.(1)General situation:The average age of patients is 59.00 ± 12.28 years old,of which the maximum age is 75 years and the minimum age is 20 years;the average disease duration is 10.32 ± 7.50 years,with the longest duration of 40 years and the shortest 0.5 years;the average BMI is 23.60 ± 3.39kg/m2,with a maximum of 31.2 kg/m2 and a minimum of 15.6 kg/m2;83.33%of patients in this study were associated with other diseases.Hypertension(53.33%)is more common.(2)Analysis of main symptoms and tongue pulse results:This study includes 22 patients(73%)with moderate pain,6 patients(20%)with severe pain,and 2 patients(7%)with the most pain:except for pain,limb numbness,insomnia,cold limbs,unfavorable walking,fatigue,constipation,blurred vision and swollen limbs are other common symptoms of the study patients;tongue is mainly dark red,tongue coating is mainly yellow,thick and greasy,the base of tongue is mainly stasis,and the pulse is mainly string,number,tough and sink.(3)Analysis of outcomes:The NRS pain scores in patients after 3 months treatment were significantly different from those in 0 month(P<0.001),and NRS scores continued to decrease after 6 months treatment,and were significantly different from 0 and 3 months(P<0.001);Diffraction analysis:The NRS pain scores in patients with moderate pain after 3 months treatment were significantly different from those in 0 month(P<0.001),and NRS scores after 6 months treatment were significantly different from 0 and 3 months(P<0.001);The NRS pain scores in patients with severe pain after 3 months treatment decreased,but were no significantly different from those in 0 month(P>0.05).The NRS scores after 6 months treatment were significantly different from 0 and 3 months(P<0.01);other symptoms also improved;for some patients with incomplete data records,only 18 patients were analyzed for HbA1c,and HbA1c(6.67±1.14)decreased after treatment(6.58±1.09).However,the difference was no statistically significant(P>0.05).(4)Safety analysis:no adverse drug reaction records and abnormal liver and kidney function were recorded in patients during the treatment period.(5)Analysis of prescriptions:Millettia,Astragalus,cassia twig,Paesmia lactiflora,Ginger,Coptis,Panax Notoginseng,tuber fleeceflower stem,Aconitum,licorice,anemarrhenae,kudzuvine root,Rhubarb,frankincense,myrrh,leech,zedoary rhizome,ligustici,epimedii herba and angelica are the most commonly used drugs for the treatment of this disease.Among them,Millettia,Astragalus,cassia twig and Paesmia lactiflora are used most frequently.The frequency was 91.11%,85.56%,73.33%and 55.56%respectively.Distribution analysis:except for Millettia,Astragalus,cassia twig and Paesmia lactiflora,patients with moderate pain also used Panax Notoginseng,tuber fleeceflower stem,etc.usually,and patients with severe pain also used Aconitum,licorice,etc,more often.Conclusions:(1)Under the intervention of traditional Chinese medicine,the pain of patients with PDPN are significantly relieved,and the degree of remission is significant with the increase of treatment course,and other symptoms are also relieved to varying degrees;(2)This study is not yet able to evaluate the effectiveness of Chinese medicine on blood glucose;(3)Millettia,Astragalus,cassia twig,Paesmia lactiflora are the core competencies for the treatment of this disease and patients with varying degrees of pain have different compatibility on this basis.Patients with moderate pain add Panax Notoginseng,tuber fleeceflower stem,etc,and patients with severe pain add Aconitum,licorice,etc.(4)It is necessary to grasp the main pathogenesis of diabetes in different periods,to treat blood sugar in stages,and promote blood circulation to remove obstruction in early and whole process,which will prevent and delay the occurrence and development of diabetic microvascular complications.
