IntroductionCervical disc degeneration is a physiological phenomenon associated with age growth,the incidence of cervical spondylosis has increased gradually with the development of society,the aggravation of the aging population and the change of people's work and life style,it has become one of the common and frequently occurring diseases of the clinic.On the one hand,The technology of early detection of intervertebral disc by MRI is becoming more and more mature,It not only provides accurate judgment and choice of treatment for clinicians,but also has high scientific research value.Using MRI to detect intervertebral disc degeneration,it can evaluate the degree of degeneration of intervertebral disc,nonionizing radiation,and has no influence on the physiological state of human body.Because of MRI's high soft tissue contrast,MRI can not only observe the histological structure of the intervertebral disc,display the lesions of the intervertebral disc itself,peripheral nerve roots,spinal canal,but also evaluate the biochemical changes of the intervertebral disc.MRI can clearly show the dehydration changes in the nucleus of the nucleus,which is mainly manifested in the decrease of the signal in the T2 intervertebral disc.At present,a small number of classification standards or classifications can indirectly reflect the degree of intervertebral disc tissue and degeneration,but there are certain deficiencies.On the other hand,Cervical disc degeneration is the pathological basis for intervertebral disc herniation and many spinal diseases,the main manifestations of disc degeneration include:the number of nucleus pulposus cells decreased and decreased function of biochemical components of extracellular matrix changes and cells,reduction of macromolecules such as proteoglycan,type ? collagen(Collagen ?)and other macromolecules,intervertebral disc stress change,intervertebral height loss,which eventually led to the loss of loss of mechanical function.Some studies have demonstrated that CTGF/CCN2,BMP-2 and BMP-7 play an important role in the synthesis of extracellular matrix in intervertebral disc.It is found that CTGF/CCN2,BMP-2 and BMP-7 participate in the process of intervertebral disc development from early stage to intervertebral disc degeneration.Recent studies showed that CTGF/CCN2,BMP-2 and BMP-7 increased in degenerative intervertebral disc,and CTGF/CCN2,BMP-2 and BMP-7 played an important role in degenerative process.ObjectiveAt present,the imaging classification of cervical disc degeneration is rare,it is less that the degree of degeneration of intervertebral disc is evaluated by combining imaging with pathology.By observing the Miyazaki grading and the degeneration of the cervical intervertebral disc,BMP-7 biochemical BMP-2,CTGF correlation,and carries on the research of relationship between different reaction to determine the extent of degeneration of cervical intervertebral disc degeneration in the process of molecular and imaging,through the evaluation of cervical MRI intervertebral disc degeneration degree in clinical,provide a relatively reliable opportunity for the biological treatment of intervertebral disc.Methods1.The patienes who will have anterior cervical spine surgery routinely underwent cervical 1.5T MRI examination in our hospital,the degenerative evaluation system of cervical disc degeneration was performed by using the MRI degeneration test system by selecting cervical sagittal and transection images of T2 images.The Miyazaki grading system is used in the hierarchical system(Table 1).Each patient was evaluated by MRI grading responsibility segments in our department including the surgeon,3 above deputy chief physician,and photographed.2.By the same surgeon for anterior cervical spine surgery,2-3 intervertebral disc patients were collected during operation,remove the disposable disc front and middle part of nucleus nucleus,collecting disc using saline wash blood,immediately in formalin solution(concentration 10%)for tissue fixation.The glassware and equipment needed for the day before the experiment at 180 degrees C high temperature baking 6 hours has reached the purpose of eliminating the RNA enzyme.After paraffin embedding and slicing,HE staining,immunohistochemical staining of BMP-2,immunohistochemical staining of BMP-7,and immunohistochemical staining of CTGF were performed respectively.ResultsThe immunohistochemical results of three different indexes of BMP-2,BMP-7 and CTGF were different from the correlation between the MRI classification of cervical intervertebral disc and the correlation with the different cervical segments.In BMP-2 immunohistochemical staining,the cumulative light density(IDO)was correlated with the MRI classification of the cervical intervertebral disc(P<0.05);The cumulative optical density(IOD)was correlated with the realisation of segments in different cervical vertebrae(C4-5,5-6,6-7).(P<0.05);The correlation between the nuclear count(Samples)and the MRI classification and segment of the cervical disc was correlated(P<0.05).The cumulative light density(IDO),nuclear count(Samples)and the scatter plot of the MRI classification of the cervical intervertebral disc showed a tendency to rise.In experimental BMP-7 immunohistochemical staining,there was no correlation between cumulative optical density(IDO),nuclear count(Samples)and Ping Junguang density(Density(mean))with cervical intervertebral disc MRI grading and cervical segment.But in the scatter plot,the cumulative optical density(IDO),nuclear count(Samples),mean light density(Density(mean))of BMP-7 showed an upward trend with the MRI grading of cervical disc.In experimental CTGF immunohistochemical staining,cumulative optical density(IDO)and mean light density(Density(mean))were not correlated with MRI grading of cervical disc.;The negative correlation between the cumulative light density(IDO)and the different cervical segments(correlation coefficient of-0.440,P<0.05);However,in the scatter plot,the CTGF cumulative light density(IDO)and the MRI classification of the cervical intervertebral disc showed a downward trend.Conclusion1.Miyazaki classification of cervical vertebra MRI can be a good judge of the degree of cervical disc degeneration,and it can classify the degree of degeneration of the cervical intervertebral disc.2.There is a positive correlation between the Miyazaki classification of cervical vertebra MRI and BMP-2.In physiological state,the content of BMP-2 increases with the degree of degeneration of intervertebral disc.3.The Miyazaki classification of cervical vertebra MRI has no correlation with BMP-7,but with the degree of degeneration of intervertebral disc,the content of BMP-7 has a tendency to rise.4.The Miyazaki classification of cervical vertebra MRI has no correlation with CTGF,but with the increase of the degree of degeneration of intervertebral disc,the content of BMP-7 has a tendency to decrease.
