BackgroundIdiopathic membranous nephropathy(IMN)is one of the most common pathological types of adult primary nephritic syndrome.Currently,podocyte injury is widely considered to be the core mechanism of idiopathic membranous nephropathy.Nephrin and podocin are involved in the formation of filtration barriers and signal transduction,and when they are caused by various factors,they lead to the production of proteinuria.In recent years,a large number of studies have confirmed that podocytes may be stimulated by TGF-?1 stimulation.The occurrence of apoptosis and epithelial-mesenchymal cell transdifferentiation and other diseases,leading to changes in cell phenotype,loss of its original morphological structure and highly specific function,proteinuria occurs.In addition,Chinese medicine has provided more effective methods for the treatment of IMN based on syndrome differentiation and treatment,significantly improved the clinical remission rate,reduced the adverse reactions of Western medicine treatment programs,improved the quality of life of patients,and showed the clinical application of Chinese medicine in the treatment of IMN.Value and unique advantages.At present,it is believed that the pathogenesis of this disease belongs to this virtual standard,and the basic pathogenesis of spleen and kidney deficiency and dampness and blood stasis has been widely recognized.Feng Xie is an important pathogenic factor in the pathogenesis of this disease and it is persistent throughout,but the important role of Feng Xie in the pathogenesis of IMN has not yet received widespread attention.Instructor Professor Wang Baokui founded the Fuzhengqufengfang.The clinical observation has shown that the curative effect is definite.The study of animal experiments also showed that this party can improve the foot process fusion of podocytes in membranous nephropathy rats and thus play a role in podocyte protection.The mechanism of cell protection has not yet been clarified,so this experiment will explore the mechanism of Fuzhengqufengfang's protection of podocytes at the molecular level.PurposeThrough animal experiments,the effect of Fuzhengqufengfang on the proteinuria,renal pathology,and the expression of podocyte-related proteins nephrin,podocin,and TGF-?1 in rats with membranous nephropathy was observed,and the effect mechanism of Fuzhengqufengfang was determined.Methods1,Forty-five SPF healthy male Sprague-Dawley rats aged about 6 weeks and weighing 160-180g were used.After 1 week of adaptive feeding,the 24-hour urinary protein quantification was negative,and then randomly divided into 15 normal groups.Group 30 only.2,In the model group,the modified Border method was used to establish a rat model of membranous nephropathy by tail vein injection of cationized bovine serum albumin.24 hU-Pr>20 mg confirmed the successful model establishment.3,The model group rats were randomly divided into model control group and traditional Chinese medicine treatment group.The Chinese medicine treatment group was treated with Fuzheng Qufeng Fang.The model control group and normal group were given equal volume of deionized water for 4 weeks.4,Observe the general condition of the rats in each group,and detect 24 hU-Pr after 4 weeks of gavage;After the end of gavage treatment,each group of kidney tissue was taken and the pathological changes of the kidney tissue were observed under light microscope and electron microscope.The distribution and expression of nehprin,podocin and TGF-?1 in renal cortex were detected by immunohistochemistry and Western Blot.5,using SPSS22.0 data statistics,P<0.05 said that the difference was statistically significant.Results1,24 hours urine protein quantificationThe proteinuria of rats in model control group and traditional Chinese medicine treatment group was significantly higher than that in normal group after modeling(P<0.01);after 4 weeks of traditional Chinese medicine administration,protein levels in the model control group and Chinese medicine treatment group were still significantly higher than normal.Group(P<0.01),but the Chinese medicine treatment group was significantly lower than the model control group(P<0.05).2,Kidney histopathological observationLight microscopic observation:Compared with the normal group,the model control group showed an increase in glomerular volume,mild degree of basilar membrane hyperplasia,vacuolar degeneration,thickening of renal tubular epithelial cells,mild degeneration and vacuolar degeneration,Visible protein tube type,Chinese medicine treatment group basement membrane thickening and renal tubular epithelial vacuolization degeneration.Electron microscopic observation:The morphology of glomerular podocytes in the normal group was normal.The foot process was extensively fused in the model control group.A large number of electron dense deposits were seen on the epithelial side.The basement membrane was thickened and diffused.The basement membrane of the Chinese medicine treatment group was slightly thickened.Convergence reduces.3,Distribution and expression of nehprin,podocin and TGF-?1 in renal cortexCompared with the normal group,the nephrin staining of the model control group changed from a continuous linear distribution to a diffuse granular distribution along the capillary loop.The nephrin positive staining of the Chinese medicine treatment group showed an uneven continuous line distribution;the nephrin protein was detected in each group.Compared with the normal group,the expression of nephrin protein in the glomeruli of the model control rats was significantly decreased(P<0.05),and the expression of nephrin protein in glomeruli of the rats treated with Chinese medicine was higher than that of the model control group(P<0.05)was statistically significant.Compared with the normal group,the positive staining of podocin and TGF-?1 in the model control group changed from a continuous linear distribution to a diffuse patchy distribution along the capillary loop.The staining was aggravated.The podocin and TGF-?1 along the capillary loop were observed in the traditional Chinese medicine treatment group.Inhomogeneous continuous line-like distribution;compared with the normal group,the expression of podocin and TGF-?1 protein in glomeruli of the model control rats was significantly increased(P<0.05),and the rats in the Chinese medicine treatment group had glomerular The expression levels of podocin and TGF-?1 protein were significantly lower than those in the control group(P<0.05).ConclusionFuzhengqufengfang can reduce proteinuria in membranous nephropathy rats,reduce pathological injury and achieve the protection of podocytes in membranous nephropathy rats by regulating podocyte-related proteins nephrin,podocin and TGF-?1.1.Fuzhengqufengfang can reduce proteinuria in C-BSA model rats.2.Fuzhengqufengfang can reduce the renal pathological damage in C-BSA model rats.3.Fuzhengqufengfang can up-regulate the distribution and expression of nephrin protein of rat podocytes in C-BSA model.4.Fuzhengqufengfang can down-regulate the expression of podocin protein in rat podocytes of C-BSA model.5.Fuzhengqufengfang can reduce the distribution and expression of TGF-?1 protein in rat podocytes of C-BSA model.
