Research Background:DM is caused by a combination of genetic and environmental factors.A group of clinical syndromes characterized by abnormal glucose metabolism, characterized by abnormal glucose metabolism,is a frequently-occurring and common disease. In recent years, the incidence of diabetes has been increasing year by year, and is becoming younger. The prevalence of diabetes in China is 10.9%.The prediabetes rate is as high as 15.5%and the prevalence rate is less than 5.9%..Diabetes,as a chronic disease, has a great impact on the physical and mental health of the patient, especially the complications, which is a serious decline in the patient's quality of life.Sleep disorders,Chinese medicine is called "nothing",its clinical manifestations are divided into difficulty falling asleep, easy to wake up,many dreams, etc.,have a great influence on people's daytime normal work, life, learning, emotions and health.Sleep disorder is a common concomitant symptom of diabetes. In recent years, both Chinese and Western medicine have paid more and more attention to diabetes and insomnia. The two affect each other and poor control can cause nausea.Sleep disorder is a more stubborn disease, and diabetes combined with sleep disorders is more difficult to control. In the treatment of diabetes insomnia,Western medicine is mainly dominated by sedative and hypnotic drugs.Although Western medicine has seen more and more researches in recent years, the effect is not satisfactory. In traditional Chinese medicine, more methods are used.In addition to traditional Chinese medicine, acupuncture, massage, auricular acupuncture, regimen, etc, have good results.Accurate dialectic plays a crucial role in treatment. Therefore, providing objective quantitative indicators for dialectical diagnosis of diabetic insomnia has gradually become a research hotspot, and there has not been too much detailed research on the differences in blood glucose control levels between syndromes.Based on this, the author quantifies the TCM syndrome type of diabetes, explores the relationship between medical syndrome and insomnia, and the correlation with blood glucose.Research Objectives:This topic is under the guidance of TCM theory, the study of diabetes associated with sleep disorders, statistics the incidence of sleep disorders in hospitalized patients, hope that through this study to explore the characteristics of diabetes associated with sleep disorders syndromes, provide guidance to Chinese medicine treatment of diabetes associated with sleep disorders, and explore the diabetes associated with sleep disorders and the relationship between blood glucose control,so as to cause clinical value, improve the accuracy of the syndrome differentiation and clinical curative effect,in order to improve the quality of life of patients with diabetes.Research Methods:1.116 patients with type 2 diabetes admitted to the Department of Endocrinology at Dong fang Hospital of Beijing University of Chinese Medicine from May 2017 to December 2017 were selected as the study subjects.2.According to the Pittsburgh Sleep Quality Index scale and the Diabetic Clinical Observation Table, case collection and questionnaire survey were conducted.General conditions include name, gender, age, height, weight, duration, and treatment.Laboratory indicators include fasting blood glucose, postprandial 2-hour blood glucose, glycated hemoglobin, fasting insulin, and blood lipids.Record patient symptoms, tongue coating, pulse and other TCM syndromes.TCM Syndrome Differentiation According to the Diagnostic Criteria for Diabetes TCM Syndrome in Guiding Principles for Clinical Research of New Chinese Medicines, all patients are divided into:Yin deficiency heat Sheng syndrome,Damp heat syndrome,Qi and Yin deficiency syndrome,Yin and Yang deficiency syndrome, and blood Weary syndrome 5 syndromes. All data were statistically analyzed using SPSS20.0.Measured data were expressed as mean±standard deviation.T-test and analysis of variance or non-parametric test were used.Chi-square test or rank sum test was used for count data.Research Result:1.Of the 116 patients with type 2 diabetes in this study, women were more likely to suffer from sleep disorders than men,and there was no significant difference in age.In terms of disease course, the prevalence of>10 years was significantly higher than<10 years;A study of BMI found that BMI was negatively correlated with sleep quality;index comparisons Fasting blood glucose was positively correlated with PSQI scores in patients with sleep disorders;insulin resistance in the sleep disorder group was significantly higher than in the group without sleep;postprandial Blood glucose and HbA1C did not suggest a correlation;in addition, there was a correlation between LDL in blood lipids and the occurrence of sleep disorders.2n In this group of 116 patients, there were 48 cases of deficiency of both qi and yin with an incidence rate of 41.38%;followed by yin and yang deficiency, blood stasis, collaterals, damp heat, spleen, and yin deficiency. There were 28 cases of type 2 diabetes with sleep disorders with deficiency of qi and yin accounting for 40.58%;followed by yin and yang deficiency in 20 cases, accounting for 28.99%;blood stasis and phlegm type in 9 cases, accounting for 13.0%;dampness and spleen deficiency in 7 cases, accounting for 10.14%;Yin deficiency heat type 5 cases, accounting for 7.25%.There was no significant difference between the two groups of constituent ratios.3.Patients with sleep disorders have different age stratification patterns in their TCM syndromes. The early stage is mostly yin-deficiency and heat-rich type.In the middle and late stages, there are mostly qi-yin deficiencies and yin-yang deficiency types. The oldest person is the blood stasis venation network.Type;disease course comparison, the longest duration of yin and yang deficiency type. This study showed that there was no significant correlation between the severity of sleep and TCM syndrome types, but there was a significant difference in the PSQI scores among the syndromes associated with sleep disorders. The PSQI score of damp-heat trapped spleen was 14.57±4.24,followed by yin and yang deficiency. The type score was 14.50±2.98.4.This study shows that patients with diabetes insomnia have differences between the medical type and glycohemoglobin and insulin resistance.Among them, glycosylated hemoglobin of yin and yang deficiency type is relatively high, while insulin resistance of qi and yin deficiency type is more obvious.Analysis Conclusion:1.There is a correlation between FBG,HOMA-IR,LDL and diabetes insomnia. It may be a risk factor of T2DM with sleep disorders and participate in the kidney development of diabetes with sleep disorders.2.The onset ages of different TCM syndromes are different, and deficiency of both qi and yin is the common syndrome type.Yin and Yang deficiency are more common in the late stage of disease. The mean age of blood-throat-inflammation-type patients is the highest;the PSQI scores among various syndromes exist.Significant differences,of which the hot and damp spleen type score is higher, followed by yin and yang deficiency;at the same time T2DM with sleep disorder TCM syndrome type and glycemic control, insulin resistance have a certain correlation.
