Research 1 Professor Xu Xuying’s Experience in Treating Chronic WoundsResearch purposes:The system explores,collates,summarizes,and inherits the academic ideas and clinical experience of Prof.Xu Xuying’s treatment of chronic skin ulcers to improve my theoretical level of Chinese medicine and Chinese medicine diagnosis and treatment technology.Research methods:Through learning from the teacher,copying the prescriptions,recording medical records and learning experience,collecting and organizing Xu’s medical papers,combining the clinical research progress of modern medicine and traditional Chinese medicine in the treatment of chronic skin ulcers,sum up Professor Xu Xuying’s academic ideas and clinical experience in treating chronic skin ulcers.Research result:Summing up Professor Xu Xuying’s academic ideas on treating chronic skin ulcers includes the following three aspects:(1)Pay attention to the overall concept of Chinese medicine;(2)treat from the blood;(3)solidify the spleen and stomach.Summing up Xu Xuying’s clinical experience in treating chronic skin ulcers includes the following three aspects:(1)"wet","blood stasis" and "deficiency" are the three major pathological factors of chronic skin ulcers,among which "Dampness" is the external factor,and "blood stasis" and "deficiency" are the essential factors.(2)The principle of treatment advocates that "supplementation of weakness and removing stasis" is the main," removing dampness" is auxiliary.The treatment should emphasize tonification and purgation in combination,regulating Qi-blood at the same time,warming yang meridian activating and compatibility of drug properties.The effect is obvious.Research conclusion:Professor Xu Xuying applied the method of removing stasis and damp,supplementing deficiency method in the treatment of chronic skin ulcers,which has significant clinical effects and deserves to be learned and passed down by young physicians.Research 2 Effect of Huiyang Shengjiu Ointment on Macrophage of Chronic WoundsResearch purposes:Observe the effect of Huiyang Shengji ointment on chronic wound macrophages and explore the mechanism of Huiyang Shengji ointment in promoting chronic wound healing.Research methods:64 patients with chronic skin ulcers were randomly divided into the treatment group and the control group,with 32 cases in each group.The treatment group was given Huiyang Shengji ointment to cover the wound,and the control group was treated with Mupirocin Ointment to cover the wound,the course of treatment was fourteen days.In the zeroth,third,seventh and fourteenth days of treatment,the central granulation of the wound and the secretion of the wound were extracted.Immunohistochemistry was used to detect the number of macrophages in the wound tissue(CD68 positive cells),Ml macrophages(CD86,i-NOS positive cells),M2 macrophages(CD206,Argi positive cells).The tumor necrosis factor-alpha(TNF--alpha)and interleukin 10(IL-10)secreted by macrophages in the wound were measured by ELISA.Research result:(1)Baseline comparison:there was no significant difference in age,sex and duration between the two groups before treatment(P>0.05),and the two groups were comparable.(2)Inter group comparison of wound macrophage and cell factor in secretion:after third,seventh,fourteenth days of treatment,the number of CD68 positive cells in the treatment group was significantly higher than that in the control group(P<0.05).On the third,seventh day of treatment,the number of CD86 and i-NOS positive cells and the level of TNF-alpha in the treatment group were significantly higher than those in the control group(P<0.05).After treatment for fourteenth days,the number of CD206 and Argi positive cells and IL-10 level in the treatment group were significantly higher than those in the control group(P<0.05).Intra group comparison:the number of CD86 and i-NOS positive cells and the level of TNF-alpha in the treatment group increased significantly at third,seventh days(P<0.05),the number of CD86 and i-NOS positive cells and the level of TNF-alpha were significantly decreased in Fourteenth days(P<0.05),the number of CD206 and Argi positive cells and the level of IL-10 increased significantly(P<0.05 or 0.01).Research conclusion:The mechanism of Huiyang Shengji ointment to promote the healing of chronic wounds may promote the activation,phenotype transformation and secretion function of macrophages,and its effect is better than Mupirocin Ointment.
