Objects:1.To investigate the feasibility of early detection of brain changes in mild cognitive impairment by using arterial spin labeling combined with CO2 inhalation test.2.Use arterial spin labeling combined with CO2 inhalation test to explore mild cognitive impairment phlegm obstruction syndrome and kidney essence deficiency syndrome, initially explore the relationship between different types of the TCM syndrome and cerebral blood flow,in order to providing objective indicators to distinguish different syndrome types of mild cognitive impairment.Materials and Methods:The patients with mild cognitive impairment were from the encephalopathy clinic of the Beijing university of Chinese medicine third affiliated hospital, among them there are 10 patients with phlegm obstruction syndrome,10 patients with kidney essence deficiencysyndrome. And 10 healthy persons were included in the study.Using the GE discovery 7503.0T MRI scanner, arterial spin labeling examinations were performed on two groups under resting state and CO2 loading conditions. After inspection,we used the ADW4.6 post-processing work station and the Functool software to deal with the images. The following regions were selected as ROIs:the bilateral frontal gyrus, caudate nucleus head, lenticular nucleus, superior temporal gyrus, and their cerebral blood flow(CBF)were measured. The CBF before and after CO2 inhalation test was measured. Then SPSS 22.0 software was used for statistical analysis. The CBF of patients with mild cognitive impairment and healthy subjects were compared and the early brain changes were explored, and the CBF of the phlegm obstruction syndrome and kidney essence deficiency syndrome were compared.Results:1.The CBF of each ROI in the mild cognitive impairment group increased after CO2 inhalation test compared with the resting state, with statistically significant differences.(P<0.05)2.The CBF of the ROIs in the control group increased after CO2 inhalation test compared to the resting state, with statistically significant differences.(P<0.05)3.The CBF of each ROI in the mild cognitive impairment group decreased compared with the CBF of the corresponding ROI of the control group, with statistically significant differences.(P<0.05)4.In the resting state, there was no difference between phlegm obstruction syndrome and kidney essence deficiency syndrome, among the mild cognitive impairment group.(P>0.05)5.In the resting state, the CBF of the ROIs of the female brain in the control group were higher than those of the male, with statistically significant differences.(P<0.05)Conclusion:1.The combination of ASL and CO2 inhalation test can detect cerebral blood vessel reactivity in normal and mild cognitive impairment patients.2.Cerebral blood flow in patients with mild cognitive impairment decreased compared with the control group.3.In the resting state, the CBF of brain ROI in patients with MCI phlegm obstruction syndrome was not different from that in patients with kidney essence deficiency syndrome.4.In the resting state, the CBF of women's ROI in the control group is higher than that of men.
