Abstract/Summary:
Objective:To investigate a quantitative analysis of the change of cerebral blood flow perfusion changes in obstructive sleep apnea hypoventilation syndrome disease(OSAHS)patients in the resting state using arterial spin labeling(ASL)technique,further explore and analyze the relationship of the change of cerebral blood flow perfusion in patients with OSAHS and the brain function.Methods:1.Choose 30 patients with severe male OSAHS patients in our hospital respiratory clinic between august 2013 and january 2016,and 30 age-matched and years-of-education matched male good sleepers(GSs)were included in this study.The pseudocontinuous arterial spin labeling(PCASL)data of resting-state were acquired and preprocessed.The regional cerebral blood flow(rCBF)was obtained according to the processing of data.2.On the day of the brain functional data scan,all participants were evaluated by Montreal cognitive assessment(MoCA)and Epworth sleepiness scale(ESS).3.The preprocessing of functional images were by means of the ASL-tbx software on the Matlab 2010 a station,the rCBF value between groups were compared respectively by two sample t test with REST software,then the brain areas which are statistically significant were obtained;then Pearson correlation analysis for regions with abnormal rCBF value with Clinical scale Score were performed respectively using IBM SPSS19.0 statistical software.Results:1.The age and education level in the OSAHS group were almost the same with those in the GSs group,there were no statistically significant difference(P>0.05);the percentage of rapid eye movement sleep stages in the total sleep time,total sleep time and lowest SaO2 of OSAHS group were significantly lower than that of the GSs group,AHI,SaO2< 90% value and micro-arousal index were significantly higher than the GSs group(P<0.05).2.Compared with GSs,participants with severe OSAHS had reduced rCBF in the left cerebellum posterior lobe,left temporal lobe,right medial frontal gyrus,and bilateral parahippocampal gyrus and increased rCBF in the bilateral superior frontal gyrus.3.The lower mean CBF in the right parahippocampal gyrus exhibited a significant positive correlation with arousal index(r=0.365,P=0.047).The increased mean CBF in the left superior frontal gyrus exhibited a significant positive correlation with the longest apnoea time(r=0.422,P=0.020),in the right superior frontal gyrus exhibited a significant positive correlation with the longest apnoea time(r=0.447,P=0.013).Conclusions:1.The OSAHS patients exist changes of rCBF in the left cerebellum posterior lobe,left temporal lobe,right medial frontal gyrus,bilateral parahippocampal gyrus and bilateral superior frontal gyrus.2.The rCBF value in abnormal perfusion regions were related with the Clinical scale Score,thus speculating that the rCBF change in these brain areas is likely to be one of the main mechanisms of clinical symptoms generating in OSAHS patients.
