Background:Breast cancer is top one cancer in women with high incidence all over the world and a great threat to women's physical and mental health.In recent years,the prevalence of breast cancer increased year by year,but with the level of earlier screening and clinical diagnosis and treatment progress,breast cancer survival rate increased,prolonged survival getting more.The concept of breast cancer rehabilitation is getting more and more attention.Surgery is priority to the clinical treatment of breast cancer,combined with auxiliary put,chemotherapy and endocrine therapy and so on.A series of dysfunction,including local or systemic problems,such as limited neck/ shoulder pain,joint activities,decreased muscle strength,fatigue,upper limb lymphedema,neuropathy,hormone disorder psychological problems presents post-operation.Therefore,not only breast cancer treatment itself is concerned about,its postoperative dysfunction has become the focus of attention.Among them,the neck and shoulder pain and dysfunctions not only affect the daily life and the life quality of patients,but can increase the risk of other postoperative complications also.Although there are some reports about evaluation and treatment neck and shoulder pain dysfunction post-operation of breast cancer in oversea country,the assessment methods,standards,and postoperative follow-up time was not confirmed,otherwise the treatment options are not the same.the studies about assessment and evaluation on the incidence and risk factors for neck and shoulder pain dysfunction are still quite different.Objective:The purpose of this study is assessment and evaluation on the incidence of neck and shoulder pain,dysfunction in breast cancer survivors post-operation,and to explore the related factors.Effective evaluation method of neck and shoulder dysfunction after breast cancer is preliminarily explored in order to provide research rationale for prevention and treatment.There are certain positive significance to alleviate the patient's pain and psychological mood and restore physical function and quality of life.Method:57 patients who underwent radical surgery for breast cancer were routinely collected based on the results of the questionnaire survey.The subjects were divided into pain group(N = 33)with postoperative neck and shoulder pain and dysfunction and non-pain group without any neck and shoulder pain and dysfunction(N = 24).Two groups of subjects were measured in neck,shoulder joint motion,upper trapezius TMG,resting sEMG,M a x i ma l Vo l u n t a r y C o n t r a c t i o n(MVC),30% MVC fatigue test.Chi-square test and independent samples T test was used for some parameters in the experimental group,while multiple linear regression analysis is used for multivariate analysis for both group.P <0.05 was set a statistically significant difference level.Result:The survivors of patients with breast cancer has the 57.9%incidence of neck and shoulder pain and dysfunction.Age,SPADI scores were risk factors for neck and shoulder pain after radical mastectomy,the greater the age of the patients,the greater the likelihood of postoperative neck and shoulder pain(P<0.05);Similarly,the higher the SPADI scores,the greater the likelihood of postoperative neck and shoulder pain(P<0.05);The range of horizontal adduction of the shoulder in the patient was the predicting factor of neck and shoulder pain after radical mastectomy(P<0.05).The higher the angle of range of horizontal adduction in the shoulder,the greater possibility of the postoperative neck and shoulder pain.Conclusion :About sixty percent of breast cancer survivors with neck and shoulder pain and dysfunction after radical surgery was founc.Age,upper limb edema,the level of shoulder involvement,SPADI scale score,and the contraction and fatigue of the upper trapezius muscle were related factors of postoperative neck and shoulder pain.Patient's age,joint mobility,and shoulder dysfunction are independent risk factors for neck and shoulder pain dysfunction.
