Serum SDF-1/CXCR4 Levels In Patients With Type 2 Diabetes Mellitus And Osteoarthritis And Their Clinical Value

Objective:To investigate the relationship between type 2 diabetes mellitus associated osteoarthritis and clinical biochemical index.And discuss the risk and protective factors of type 2 diabetes mellitus associated osteoarthritis,in order to provide evidence and direction for the strategy of prevention and treatment for the disease.Method:Patients treated in diabetes department,sport medicine department and orthopedics department of the first affiliated hospital of kunming medical university from January 2015 to December 2016 were randomly selected,including 200 cases with type 2 diabetes(DM group),97cases with osteoarthritis(OA group),83 cases with both diseases(DM and OA group)and 200 normal controls(NC group)from the examination center in the same period.Patients' gender,age,hight,weight,blood pressure,BMI,blood glucose,fasting insulin,fasting C peptide,blood fat,calcium and other biochemical indicators were recorded.All data were analyzed with SPPS21.0 statistical package.The normal distribution data depressed as mean ±standard deviation(x±S),non-normal distributions as median ±[P25%,P75%].Variance analysis was used between groups and LSD-t test for comparison between two groups.Logistic regression analysis and Pearson correlation analysis were used to analysis the correlation.Using P =0.05 as the significance test standard,P<0.05 represents statistically significant differences between groups.Results:(1)Statistical differences of indicators showed by analysis of variance between four groups are:the glycosylated hemoglobin(NC group:5.97±1.42%,DM group:7.06 ±1.76%,OA group:6.16 ±1.23%,DM with OA group:7.76 ±1.84%),HOMA-IR(NC group:0.58 ± 0.34,DM group and 1.19 ±0.78,OA group:0.85±-0.62,DM with OA group,1.46 ±0.76)and BMI(NC group:22.12 ± 2.23 Kg/m2,DM groups:25.44 ±2.83 Kg/m2,OA group:24.35± 1.89 Kg/m2,DM with OA group:26.12 ± 2.75 Kg/m2),Ca2 + concentration(NC group:2.33 ± 0.23 tendency for L,DM groups:2.02 ± 0.18 tendency for L,OA group 2.00 ± 0.16 tendency for L,DM with OA group:2.00 ± 0.18 tendency/L),P values were all less than 0.05.(2)Logistic regression analysis results suggested that glycosylated hemoglobin,HOMA IR and BMI were risk factors for diabetes associated wih osteoarthritis and Ca2+ concentration was protective factor.Conclusion:Glycosylated hemoglobinAlc,HOMA IR,BMI were risk factors for type 2 diabetes mellitus associated with osteoarthritis,and Ca2 + concentration was its protection factor.Hypoglycemic control,weight loss and appropriate supplements of calcium can prevent the onset of type 2 diabetes mellitus patients with osteoarthritis.Objective:To detect serum concentration changes of SDF-1/CXCR4 and research its association with biochemical indicators in patients with type 2 diabetes and osteoarthritis.And discuss the role of SDF-1/CXCR4 axis played in the disease to provide evidence and direction of the targeted drugs for type 2 diabetes associated variance showed statistically significant differences of SDF-1 and CXCR4 concentration between the four groups.After comparison between the four groups of SDF-1,the order from high to low is:DM combined OA group(7.765 ±1.233 ng/ml),OA group(6.645±1.237 ng/ml),DM group(5.194±0.756ng/ml),NC group(4.877 ± 0.668 ng/ml).Level of CXCR4 among four groups from high to low were DM combined OA group(78.335±3.583 ng/ml),OA group(60.801±7.284 ng/ml),DM group(50.084±7.897 ng/ml),NC group(47.910±3.729 ng/ml)and P values<0.05(2)Pearson correlation analysis showed that SDF-1/CXCR4 and BMI,glycosylated hemoglobin,serum concentration HOMA-IR were positively correlated(P<0.05),and negatively correlated with Ca2 + concentration(P<0.05);(3)Logistic regression analysis results suggested SDF-1 and CXCR4 level as risk factors for diabetes associated with osteoarthritis.Conclusion:The increasing serum concentration of SDF-1 and CXCR4 were risk factors for type 2diabetes associated osteoarthritis.They were positively correlated with BMI,glycosylated hemoglobin,HOMA-IR and negatively correlated with Ca2 + concentration.This axis may be involved in progression of osteoarthritis in diabetic patients and become the future targets for the treatment of diabetes associated osteoarthritis.
