| Diabetic kidney disease(DKD)is one of the most common chronic complications of diabetes,the incidence of early stage features urinary albumin excretion increased in mild(microalbuminuria),gradually progression to macroalbuminuria and serum creatinine levels increased,eventually renal failure.This disease is one of the main causes of death in patients with diabetes mellitus and the disabled,seriously affects the life quality of patients,increases the economic burden of patients.Therefore,to explore the effective treatment method actively is particularly important.Professor Lou Xien based on the theory of traditional Chinese medicine,combined with clinical observation,summarized that the etiology and pathogenesis of diabetic kidney disease is attributed to "Qi and yin deficiency,blood stasis and toxin blocking",and developed geqi Jiangtang granule to prevent its development early andr achieved a better clinical effect.Objective:Through a clinical trial observation of geqi Jiangtang granule used into Qi and yin deficiency,blood stasis and toxin block in early diabetic kidney disease patients with clinical symptoms,blood glucose,blood lipid,UAE,TGF-β1,MMP-9 and so on,to evaluate the clinical curative effect and discuss the mechanism of treatment of diabetic kidney disease,delaying the course of development.In order to clinical treatment guidance,provide new ideas for Chinese medicine intervention diabetic kidney disease.At the same time by observing the security test basic situation and biochemical indexes of evaluation of geqi Jiangtang granules.Methods:In order to meet the inclusion criteria of diabetic kidney disease patients as the research object,a total of 58 patients were randomly divided into treatment group(30 cases)and control group(28 cases),given the basic treatment of diabetes for 2 weeks to control blood glucose,blood pressure,blood lipids after.2 weeks into the experiment observation period,treatment group were given basic treatment + Losartan Potassium Tablets + Geqi Jiangtang granule,control group with the basic treatment of Losartan Potassium Tablets treatment.After the end of treatment with FPG,2hPG,HbAlc,blood lipid,UAE,TGF-β1 and MMP-9 as indicators of efficacy,collect test data,statistical processing,and evaluation of geqi Effect of Jiangtang granule in the treatment of early diabetic kidney disease,to explore the mechanism.Results:1.Analysis of curative effect of TCM syndrome:by statistical analysis,the two groups before and after treatment in comparison and comparison between the two groups after treatment,the differences were statistically significant(P<0.05).2.The effects on blood glucose and glycosylated hemoglobin:after treatment of the two groups of FPG,2hPG,HbAlc were significantly decreased,the difference(P<0.05);after treatment between the two groups,P<0.05,with statistical significance.3.The impact on blood lipids:the treatment group and the control group TC,TG,LDL-C were significantly lower than before treatment,HDL-C increased significantly,the difference was statistically significant(P<0.05):after the treatment,the treatment group than the control group TC,LDL decreased,HDL increased more significantly,the difference was statistically significant(P<0.05).4.Effect on urinary albumin excretion rate:compared with before treatment,the treatment group significantly decreased UAE,P<0.01,UAE in the control group also decreased P<0.05,there were significant differences between two groups;comparison between the two groups(P<0.05),the difference was statistically significant.5.The serum TGF-β1 MMP-9 effect:after treatment the treatment group and the control group of serum TGF-β1 over the previous level were decreased,P<0.05,the difference was statistically significant;while serum MMP-9 levels of two groups were higher than before,P<0.05,the difference is statistically significant.6.The total efficiency analysis:after treatment,the total efficiency of treatment group was90%;the total effective rate of control group was71.43%and P<0.05 among two groups,the difference was statistically significant.Conclusion:Geqi Jiangtang granule can effectively improve the clinical symptoms of deficiency in patients with early diabetic kidney disease of blood stasis and toxin block type control of blood glucose,blood lipid,reduce UAE,regulate TGF-β1,MMP-9,and improve renal function,protect kidney,delay the progression of diabetic kidney disease. |
