| Objective: Based on the theory of "treating the same disease with different diseases" in traditional Chinese medicine(TCM),the clinical effect of Qi Ming Granule on treating diabetic microangiopathy eye and kidney disease with deficiency of both qi and Yin was studied.Methods: 90 patients with diabetes mellitus who were diagnosed with diabetic retinopathy and diabetic nephropathy were randomly divided into 3 groups(Qiming group,Keluoxin group,combined group).On the basis of hypoglycemic,antihypertensive,lipid-lowering therapy,Qi Ming group was given oral Qi Ming granule(1 bags / time,3 times / day);Keluoxin group was given Keluoxin capsule(4 pills,3 times / day);the combined group to give Qi Ming granule and Keluoxin capsule combined(usage ibid),course in March.Monthly visits 1 times,each visit to observe the changes of visual acuity,blood pressure,fasting blood glucose,Hb A1 c,HDL-C,observe the changes of TG,Cys-C,fundus,blood beta 2-MG,24 h UTP,Scr,e GFR,TCM syndrome score index before and after treatment.Result: 1.Blood pressure,blood glucose and Hb A1c: comparison in the group,the treatment group during the blood pressure,blood sugar level basically stable,no significant difference(P>0.05);Hb A1 c before and after treatment was lower than before,the difference was statistically significant(P<0.05);comparison between groups,blood pressure,blood glucose and Hb A1 c before and after treatment there were no significant differences(P >0.05)2.The visual acuity,fundus hemorrhage changes: comparison in the group,the difference was statistically significant changes before and after the treatment of Qi Ming group and combination group(P<0.05),Keluo Xin group had no significant difference(P>0.05);comparison between groups,3 groups of patients before and after treatment were statistically significant(P<0.05).3.Patients with Cys-C,blood beta 2-MG,24 h UTP,Scr and e GFR improvement: in group comparison,the difference of Cys-C,2-MG and 24 h UTP in each group was statistically significant(P<0.05);there was no significant difference in Scr and e GFR between the treatment group and the treatment group(P<0.05).The difference of Scr and e GFR before and after treatment in the combined group was statistically significant(P<0.05);Comparison between groups,no statistically significant differences between the 3 groups of patients before and after treatment(P>0.05);each index difference between before and after treatment showed no significant difference(P>0.05),the difference between the each indexes had no significant difference(P>0.05).4.The TCM syndromes and syndrome curative effect: comparison in the group,each group after treatment of TCM syndromes and treatment significantly reduced,with statistical significance(P < 0.001);Qi Ming group was 70%,keluoxin group was 68%,the effective rate was 74%.There was no significant difference between the 3 groups of TCM syndromes(P > 0.05).5.Improvement of DR and DKD in each group: In group comparison,the DR effect was improved,The effective rate of Qi Ming group was 81.5%,the effective rate of keluoxin group was 53.6%,and the effective rate of combined group was 85.2%.There was significant difference between the 3 groups in the therapeutic effect of DR(P<0.05);DKD efficacy improvement,The effective rate of Qi Ming group was 70%,the effective rate of keluoxin group was 78%,and the effective rate of combined group was 81%.There was no significant difference between the three groups in the therapeutic effect of DKD(P > 0.05).Conclusion: 1.Qi Ming granule can reduce the fundus and visual impairment of diabetic retinopathy,but also can improve the level of Cys-C,2-MG and 24 h UTP in diabetic kidney disease.2.Effect of Qi Ming Granule on diabetic microangiopathy,kidney and eye disease patients significantly,embodies the advantages of Chinese medicine treatment of "different diseases" of Chinese medicine theory and the multiple targets,provide the basis for theprevention and treatment of diabetes medication of TCM microangiopathy,reduce diabetic microangiopathy,kidney and eye disease in patients with economic burden. |
