Objective:(1)Combining with the characteristics of China's management of resident standardization training,preparation of a scientific,objective resident clinical learning environment scale,and test the reliability and validity,It can provide a scientific and effective tool for evaluating the quality of clinical learning environment in our country,and further improving the rotation quality.(2)using the scale evaluate the clinical learning environment of residents,analyze the existing clinical learning environment problems and deficiencies for the resident clinical learning environment and provide a basis for the improvement,at the same time to provide reference for the selection of intervention measures.Methods:(1)We developed the initial scale through the study of the clinical learning environment,the domestic and foreign scholars' research on the nursing and resident clinical learning environment,In combination with the expert interview method.(2)Using resident group discussion method,combined with the actual situation of residents in our country,screening indicators,initially formed the resident clinical learning environment assessment scale.(3)Using the Delphi method to 16 experts for two rounds of questionnaire survey,on-site distribution(recycling)questionnaire,please experts to evaluate each index,and to investigate the degree of familiarity with the investigation and the basis for the judge.After the end of each round of expert consultation,calculate the positive coefficient,authority coefficient,coordinate coefficient,delete the selected dimensions and entries with the boundary value method.After two rounds of Delphi method,the final outcome scale includes 6 dimensions,namely,the Teaching physicians,the training atmosphere,the training quality improvement,the training condition,the colleague relationship,the medical security,a total of 40 items;(4)We investigated the reliability and validity of the scale by using a questionnaire survey among the residents of the 8 resident training centers in Beijing,which was conducted by a total of 960 residents.The reliability test was based on the homogeneity reliability and test-retest reliability,and the validity test was based on the content validity and construct validity.(5)The investigation data of effective questionnaire were imported into spss20.0,and statistical analysis was carried out.The significant level was<0.05.Results:(1)After two rounds of the Delphi survey,expert positive coefficients was 100%,authority coefficient was 0.858,0.848.that two rounds of survey experts enthusiasm is very high,very interested in this research,and the authority of experts is better,the research results are reliable.(2)After the reliability and validity testing,six dimension of homogeneity reliability Cronbach's a coefficient is from 0.833 to 0.939,scale overall Cronbach's a coefficient is 0.975;Retest reliability is 0.912;Content validity of 0.94;Six common factor accumulation contribution rate of 67.897%.It shows that the scale has high reliability and validity.(3)In the empirical study,we get:?The total average score of the standardized learning environment of resident standardized training was divided into 3.46,and the average scores were in the range from 3.23 to 3.67.The highest score of the dimension:the colleague relationship(3.67±0.66),lowest dimension:improve the quality of training(3.23±0.92);The highest average score of the item:during the training,you and other hospitals in training residency rapport(4.04+0.64);the lowest average score of the item:you have been trained to recognize signs of fatigue(2.83+1.15).?The different gender,education,identity,training in research physician or professional degree of clinical learning environment scores were compared after the results were P>0.05,according to a =0.05 level,the difference was not statistically significant.?The resident or specialized degree graduate student for the evaluation of clinical learning environment in professional and the hospital comparative difference was statistically significant.Conclusion:(1)The experts selected in this study are representative,the dimensions and items are reasonable and the results are reliable.(2)The scale has good reliability and validity,which can be used to evaluate the clinical learning environment of resident standardized training.So as to provide a scientific and effective tool to evaluate the quality of the clinical learning environment and improve the quality of clinical teaching.(3)The residents in general had positive experiences of the clinical learning environment in the respects,the main factors influencing the clinical learning environment score is profession,training base,has no obvious relation with gender,education,training,fixed number of year.
