To Study The Relationship Between Hematuria And Pharyngitis In Chronic Nephritis From The Theory Of "pharyngeal-kidney Correlation"

ObjectiveChronic glomerulonephritis is a common and refractory chronic kidney disease.The disease,which is caused by a variety of reasons and composed of various pathological types,belongs to immune-mediated inflammatory diseases.In our country,among a variety of causes of end-stage renal failure,chronic nephritis is in the first place,accounting for 64.1%.Diagnosis and treatment of chronic nephritis can not be neglected.NF-kB is involved in the signal transduction of cell inflammatory reaction,regulates the expression of many genes,and plays a central role in the regulation of inflammatory response.At the same time,it has been proved that MCP-1 is closely related to many kinds of kidney diseases.Based on the theory of throat and kidney,this study is to explore the relationship between chronic nephritis hematuria and pharyngitis,observe the curative effect of Chinese-herb-tea,and analyze the relationship among chronic nephritis syndrome,pharyngitis,hematuria and NF-kB,MCP-1 through detection of the level of NF-kB and MCP-1.This study is to provide new methods and ideas for the clinical treatment of chronic nephritis.MethodsIn this study,150 patients with chronic nephritis(50 patients with hematuria and pharyngitis,50 patients with hematuria and without pharyngitis and 50 patients without hematuria)are selected to observe the difference among the three groups of TCM syndromes,physical indicators.31 patients with hematuria and pharyngitis are selected to be treated with herbal decoction and Chinese-herb-tea.40 patients with hematuria are selected to analyze the relationship among chronic nephritis syndrome,pharyngitis,hematuria and NF-kB,MCP-1.Results1.TCM constitution:Among 150 patients with chronic nephritisin,Qi deficiency and Yang deficiency are common.At the same time,patients with Qi-stagnation constitutions are not rare.Compared 100 patients with hematuria with 50 patients without hematuria,Qi deficiency constitutions are more and phlegm wet constitutions are less among patients with hematuria.Compared 50 patients with hematuria and pharyngitis with 50 patients without hematuria,Qi deficiency constitutions and Qi-stagnation constitutions are more among patients with pharyngitis.Qi-stagnation constitutions are more among women compared with men.2.TCM syndrome:Qi deficiency of lung and kidney syndrome(60.00%)and damp heat syndrome(32.67%)are common.Compared 100 patients with hematuria with 50 patients without hematuria,Qi deficiency of spleen and kidney syndrome,Qi deficiency of lung and kidney syndrome,Yin deficiency of liver and kidney and damp heat syndro-me are more,while Yang deficiency of spleen and kidney syndrome and blood stasis sy-ndrome are less among patients with hematuria.Compared 50 patients with hematuria and pharyngitis with 50 patients without hematuria,Yin deficiency of liver and kidney,Qi and Yin deficiency and damp heat syndrome are more,while blood stasis syndrome is less among patients with pharyngitis.The hematuria and pharyngeal symptoms are serious in Yin deficiency of liver and kidney,damp-turbidity and damp heat syndrome,3.Pharyngitis and hematuria:The number of red blood cells in urine and the number of red blood cells at high magnification among patients with pharyngitis are higher than those without pharyngitis(P<0.05).There was a positive correlation between the number of red blood cells and the scores of pharyngeal symptoms(P<0.05).4.the Curative Effect of Chinese-herb-tea:31 cases of chronic nephritis hematuria and pharyngitis with Yin deficiency,after treatment of Chinese-herb-tea,the number of red blood cells in urine,urine red blood cells at high magnification and the scores of pharyngeal symptoms are reduced(P<0.05).The results show that before and after the treatment,the number of red blood cells in urine,urine red blood cells at high magnification and the score of pharyngeal symptoms are positively correlated(P<0.05).There is also a positive correlation between symptom scores of pharyngitis and urine(P<0.05).5.NF-kappa B,MCP-1 and Chronic Nephritis:In this study,NF-kappa B in blood and urine are more among patients with pharyngitis than those without pharyngitis,but there is no significant difference in MCP-1 in blood and urine.NF-kappa B in blood and urine and MCP-1 in blood and urine in patients with damp heat syndromes are more than those of non damp heat syndromes.There is a positive correlation between the number of red blood cells and NF-kappa B in blood and urine,but there is no significant correlation between the number of red blood cells and MCP-1 in blood and urine.There is a positive correlation between pharyngeal symptom scores and NF-kappa B in blood and urine,but there is no significant correlation between pharyngeal symptom scores and MCP-1 in blood and urine.Compared with NF-kappa B in urine,the correlations among NF-kappa B in blood,the number of urinary red blood cells and pharyngeal symptom scores are stronger.Conclusions1.Among patients with chronic nephritis,Qi deficiency,Yang deficiency and Qi-stagnation constitutions are common.Compared with patients without hematuria,Qi deficiency constitutions are more among patients with hematuria.Compared patients without pharyngitis,Qi deficiency constitutions and Qi-stagnation constitutions are more among patients with pharyngitis.2.Among patients with chronic nephritis,Qi deficiency of lung and kidney syndrome and damp heat syndrome are common.Compared with patients without hematuria,Qi deficiency of spleen and kidney syndrome,Qi deficiency of lung and kidney syndrome,Yin deficiency of liver and kidney and damp heat syndrome are more among patients with hematuria.Compared patients without pharyngitis,Yin deficiency of liver and kidney,Qi and Yin deficiency and damp heat syndrome are more among patients with pharyngitis.3.There is a close relationship between pharyngitis and hematuria.The clinical cur_ative effect of treating chronic nephritis from pharynx is positive,and it has clinical v-alue.According to the theories of meridian syndrome differentiation and treating the up?per to cure the lower disease,clear the upper and arrest discharge the lower are effective.4.NF-kappa B and MCP-1 are closely related to chronic nephritis.
