TCM Syndrome Research On Thyroid Nodules

Objective:In order to explore the common syndromes of thyroid nodules and the common symptoms of syndromes,and to explore the relationship between TCM syndromes and gender,age and laboratory indicators such as TSH,TPOAb and TGAb,and to provide preliminary reference basis for the research of TCM syndrome in thyroid nodule,and to provide ideas for the clinical diagnosis and treatment of thyroid nodules.Method:In this research,we conducted a research of 98 patients with thyroid nodules in the outpatient and inpatient department of Dongzhimen Hospital,and collected the general data and laboratory indicators,such as gender,age,height,weight,thyroid-related family history,TSH,TPOAb and TGAb.And then fill the TCM syndrome score scale.At the last,we used the chi-square test,T test or wilcoxon rank sum test and Spearman method to observe the distribution of common syndromes of thyroid nodules and the correlation between the frequency of syndrome or the syndrome scores and laboratory indicators.Result:1.In this study,98 patients with thyroid nodules were mostly female,with age at 50-59 years of age and significantly lower in subjects over 70 years of age.There was no significant difference between men and women in the age of onset.Most patients with thyroid nodules did not have a family history associated with thyroid disease.In this study,the subjects with the normal weight(BMI 18-23.9)and overweight(BMI 24-28)accounted for the largest proportion,and with low body weight accounted for the least.2.Thyroid nodules of the syndrome was composed of deficiency syndromes and evidence syndromes,which consisted of a variety of syndromes elements.The syndrome with seven and above syndromes elements at the same time accounted for 66.3%,single syndrome accounted for 3.1%.The qi deficiency syndrome,yin deficiency syndrome,and yang deficiency syndrome were the commom syndromes in the deficiency syndromes.The fire syndrome was the most commom syndrome of the evidence syndromes,and then was the qi stagnation syndrome.3.More than 50%of patients with Qi deficiency in the thyroid nodules showed mental fatigue,fatigue,shortness of breath,and more than 30%of patients showed lazy words and easy to sweat.More than 50%of patients with blood deficiency in the thyroid nodules showed the dizziness and pale or yellow looking,and more than 30%of patients showed the paleness with lips and nails,and 26.3%of patients showed menstrual scarce.More than 50%of patients with yin deficiency in the thyroid nodules showed dry mouth and throat,waist and knee soft and tinnitus,and more than 30%of patients showed array hot and night sweats,and 27.5%of patients showed hands and feet heat.More than 50%of patients with yang deficiency in the thyroid nodules showed the fear of cold,waist and knee cold,and more than 30%of patients showed frequent nocturnal enuresis,not thirsty,liking hot drinks,and 21.1%of patients showed face edema.More than 50%of patients with blood stasis in the thyroid nodules showed facial pigmentation,and more than 30%of patients showed lipstick purple,or darkness,ecchymosis,sublingual vasodilatation and purple looking,and 20-30%of patients showed dysmenorrhea,menstrual color black blood clots,amenorrhea,hard and painful neck mass,dry and rough skin.More than 50%of patients with Phlegm in the thyroid nodules showed the throat in the foreign body sensation and chest tightness,and more than 30%of patients showed the neck of the oppressed or blocked and cough spit.More than 50%of patients with qi stagnation in the thyroid nodules showed sigh,belching or hiccups,and more than 30%of patients showed emotional depression,abdominal pain and the bowel sound and breast swelling and pain before menstruation.More than 50%of patients with hot card in the thyroid nodules showed irritability,and more than 30%of patients showed mouth bitter,and urine yellow,and 20-30%of patients showed sore throat,and dry stool.More than 50%of patients with wet turbidity in the thyroid nodules showed heavy body,fat,unformed and thin stool,and more than 30%of patients showed sticky feeling in the mouth.4.In this study,the positive rate of TGAb was different in different age groups(P = 0.032).The positive rate of TGAb in patients aged with 40-59 years old was higher than patients in other age groups.There was no significant difference in the positive rate of TPOAb and the abnormal rate of TSH in different age groups.5.There were some differences in the syndromes of patients with thyroid nodules of different ages.For example,the patients with 40-49 years old and 60-69 years old compared with 30-39 years old had obvious manifestations(p= 0.014 and p= 0.012).Patients who are 40-49 years old were significantly worse than those aged with 20-29 years old in the qi stagnation syndromes(p = 0.033).Patients who are 40-49 years old were significantly worse than those aged with 20-29 years old in the yang deficiency syndromes(p =0.008).Patients who are 70-89 years old were significantly worse than those aged with 40-49 years old in the phlegm dampness syndromes(p = 0.024).But the relationship between syndromes and age is not a simple linear relationship.In the frequency of syndromes,there were significant differences in the frequency of Qi deficiency in different age groups(P = 0.032).There was no statistically significant difference between the age and other syndrome scores and the frequency of syndrome elements.6.In this study,there were differences in the frequency of TPOAb with different frequency groups in the phlegm syndromes(P = 0.016).There was no significant difference in the positive rate of TPOAb and TGAb between the two groups.in different frequency groups for other syndromes.Conclusion:1.Thyroid nodules of the syndrome is more complex,consisting of the deficiency syndromes and evidence syndromes.The qi deficiency syndrome,yin deficiency syndrome,and yang deficiency syndrome are the commom syndromes in the deficiency syndromes.The fire syndrome is the most commom syndrome of the evidence syndromes,and then is the qi stagnation syndrome.TCM syndrome in thyroid nodules are performed for a variety of syndromes existing at the same time,rarely manifested as a single syndrome elements.2.This study shows that the distribution of syndromes in thyroid nodules is associated with age.There are differences in qi deficiency,qi stagnation,yang deficiency and phlegm syndrome,but it is not a simple linear correlation.3.This study finds that the degree of phlegm coagulation is different,the TPOAb positive rate is different.
