Objective:Through the wounded vertebral nailing and across two vertebral nailing treatment method for thoracolumbar fractures patients postoperative recovery do contrast imaging pattern and the related factors, to explore different ways of two kinds of treatment for thoracolumbar fracture morphology change and corresponding clinical curative effect analysis.Methods:To collect my department in March 2010 to March 201.4, select application by the wounded vertebral nailing and cross-cultural vertebral nailing the 20 cases were followed up for patients with thoracolumbar fractures of relatively complete, it can be divided into the experimental group and control group, experimental group 20 cases, control group 20 cases, two groups of patients with preoperative vertebral body height (including front and rear), injury of the vertebral segment spine Cobb Angle and the injured area ache degree had no significant difference between group; Experimental group with short segment by the injured vertebra transpedicular method, control group choose traditional short segmental cross wound vertebral nailing method, surgery performed by the same performer and assistant, nail bar system materials are sourced from the same manufacturer, and postoperative follow-up of all in the same reading X machine prints on the film of vertebral body height, measurement (leading edge and trailing edge), the injured segmental spinal Cobb Angle, screw rod system form, inquiries pain degree of patients injured area (VAS score method), the two groups of patients with preoperative and postoperative and follow-up observation comparison, the indicators at the same time, combined with clinical patients with low back pain and functional recovery were analyzed. Including:two groups of patients in four days after operation, march, June, December and take screw rod system injured vertebral fanterior the height of the edge and trailing edge, injured vertebral segment of spinal Cobb Angle, nail bar system and complete on June, two groups of patients with preoperative and postoperative wound pain when you pick up the nail stick system level (pain blurred visual grading method is used to evaluate VAS score)Results:The preoperative and postoperative observation and follow-up data analysis to get the following results:4 days after operation, in March, injured vertebral height of vertebral body (leading edge and trailing edge) is compared between group had no statistical significance (P>0.05); After surgery to take nail stick system injured vertebral segment spine Cobb Angle had statistical significance (P<0.05); After June to take nail rod system injured vertebral frontal height between group comparison was statistically significant (P<0.05); Degree of postoperative wound pain in June is no statistical significance between groups (P>0.05), the degree of injury pain when you pick up the nail stick system comparison between groups was statistically significant (P<0.05), the injured vertebral rear height in different times is no statistical significance, postoperative follow-up found across vertebral injury group two cases of fracture.Conclusion:the spinal injury vertebral nailing treatment thoracolumbar fractures can better restore fanterior flange height and correct spinal deformity, reduce vertebral height loss and screw rod system failure, can reduce the chronic pain patients injured segment, the long-term curative effect, more traditional across injured vertebral nailing treatment is its advantage more.Conclusion:The vertebral pedicle screw fixation of thoracolumbar spine fractures can better restore vertebral height and correction of spinal deformity, loss of vertebral height reduction and fixation system failure , can reduce chronic pain patients injured segment its long-term efficacy, compared with the traditional cross-vertebral pedicle screw fixation compared bigger advantage.
