OjectiveTo explore the variation of serum s P-selectin, s E-selectin, IL-6 and IL-18 levels in the patients of pre-diabetic, simple type 2 diabetes mellitus(T2DM) and T2 DM with coronary heart disease(CHD); to understand the relationship between serum s P-selectin,s E-selectin, IL-6, IL-18 level and relative factors; to investigate the expression levels of IL-6 m RNA in peripheral blood mononuclear cells(PBMCs)from T2 DM and the relationship between all relevant factors; to investigate the possible mechanism that s P-selectin, s E-selectin, IL-6,IL-18 and IL-6 m RNA accelerate type 2 diabetes mellitus.MethodsLevel of serum s P-selectin,s E-selectin, IL-6 and IL-18 was assayed by ELISA in type 2 diabetes mellitus with or without coronary heart disease(32 and 34 cases respectively),pre-diabetic(32 cases)and control group(32 cases). The expression of IL-6m RNA in peripheral blood mononuclear cells(PBMC) was measured by reverse transcription polymerase chain reaction(RT-PCR). Meanwhile BP, BMI,TG,CHO,FBS,Hb Al C,FINS, HDL-C,LDL-C,were determined in all cases as wel I as in control group.HOMA-IR was used to calculate insulin resistance index.Results(1)The serum 1evels of s P-selectin,s E-selectin, IL-6 and IL-18 in pre-diabetic,type 2diabetes mellitus with or without coronary heart disease groups were significantly higher than normal control group(p<0.01);the serum 1evels of IL-6 and IL-18 in simple type 2diabetes mellitus group were significantly higher than pre-diabetic group(p<0.01); the serum 1evels of s P-selectin,s E-selectin, IL-6 and IL-18 in type 2 diabetes mellitus with coronary heart disease group were higher than pre-diabetic group(p<0.001~0.05);the serum level of P-selectin in type 2 diabetes mellitus with coronary heart disease group were significantly higher than simple diabetes mellitus(p<0.01).(2)It was significant positive correlation between serum levels of s P-selectin,s E-selectin, IL-6 and IL-18 and these items including FINS?FBG?Hb Al C.(r=0.3970~0.6372, p<0.01).(3)The level of s P-selectin, s E-selectin, IL-6, IL-18 and IL-6m RNA had a significantly positive correlation with HOMA-IR(r=0.5917,0.4138,0.6125,0.4809,0.5875,P<0.01).(4) SP-selectin,s E-selectin and IL-6 and IL-18 were highly correlated with each other(0.5923~0.7349, p<0.01).(5) The IL-6 m RNA expression level of peripheral blood in the pre-diabetes group,simple diabetic group, diabetes and coronary heart disease group was gradually increased,and they were statistically significant(P<0.01).(6) IL-6m RNA and serum s P-selectin, s E-selectin, IL-18, IL-6 were highly positively correlated(r=0.6390 ~ 0.7814, P<0.01), and FINS, FPG positive correlation(r=0.5316,0.6974, p<0.01).Conclusions:(1)Serum s P-selectin, s E-selectin, IL-6, IL-18 in the pre-diabetes group, simple diabetic, diabetes and coronary heart disease group were highly expressed, indicating high levels of s P- selectin, s E- selectin, IL-6, IL-18 may be associated with the development of type 2 diabetes, which may play a role in the course of the disease.(2)The IL-6m RNA levels of peripheral blood in pre-diabetes group, simple diabetic,diabetes and coronary heart disease group was in a significantly increasing trend,suggesting that IL-6m RNA may be associated with diabetic and its vascular lesions.(3) SP-selectin, s E-selectin, IL-6 and IL-18, IL-6m RNA were significantly associated with HOMA-IR, suggesting that SP-selectin, s E-selectin, IL-6, IL-18,IL-6m RNA may promote the development of type 2 diabetes by insulin resistance.
