Application And Evaluation Of TCM Evaluation Scale For “patient Report Outcome” Of Chronic Obstructive Pulmonary Disease

Objective: To valuate the instruments' reliability, validity and responsibility based on the established PROs of COPDs patientsMethods: During this scale was tested, 60 cases of COPDs patients were chosen totally by strict standard of diagnosis according to the principle such as random, repetition, comparison and equilibria and etc. Each district was further divided into TCM and western medicine group(they were respectively 30 cases). COPDs patients were treated with TCM and western medicine respectively. One curative course was 28 days. Before and after treatment, respectively QOL was measured with PROs once. In addition 40 cases healthy people were tested about their QOL for finding out this instruments' ability in distinguishing between the patients and healthy people. After the data were collected, SPSS17.0 was used to processing these data, and their results were used to valuate this instruments' feasibility, validity, reliability and responsibility.Results: 1.Feasibility: This instruments' accepted rate is 100%. Its callback rate is 100%. Its profile's accomplished rate is 100%. Its items' accomplished rate is 100%. Average time of accomplished scale is 14.48±5.26 minutes.2.Validity: Content validity: Many people related with this study took part in choosing titles and their discussion. The content of the domains and items brought forward fully embodied inner significance of PROs and stroke's character about its disease, syndrome and symptom.Its items were screened out in according with procedural ways, so this instruments has a good content validity. Constructive validity: The surveyed results were true of the instruments' structure conceived by designers in study by means of factor analysis. Criterion-related validity: Correlative coefficients of this QOLISP with WHOQOL-BREF are 0.757. They have proper relativity(P<0.01).3. Reliability: Split-half reliability coefficient of total scale and each domain are all over 0.66. Cronbach's alpha coefficient of total scale and each domain are all over 0.77.4.Responsibility:A average number of each domain's score and total scale's score of healthy people are obviously over one of the COPDs patients(p<0.01).Before and after treatment, average scores and standard deviations of QOL of the COPDs patients are respectively 100.30±12.212, and 127.95±12.093,p<0.01. A average number of each domain's score of post-treated QOL has went up more and less. Post-treated QOL of each item went up more and less(p<0.05). Among all the domains, the syndromic domain of TCM changes the most while the psychological domain changes the least, Before treatment each domain's score and total scale's score of TCM and western medicine groups were close. But after treatment scores of the syndromic domain of TCM group and physical domains were markedly higher than ones of western medicine group(p<0.05~0.01);It make clear that TCM is superior to western medicine in post-treatment syndromic domain scores and physical mental and satisfaction domains of COPDs patients.. It makes clear that TCM is superior to western medicine in getting a better score in post-treatment syndromic domain and the scale. It shows that TCM is superior to western medicine in improving QOL of syndromic domain of TCM ?physical domain, etc. of COPDs patients.Conclusions: 1. Accepted, callback and accomplished rates of this instruments are very good. Its accomplished time is not long. So its clinical feasibility is good.2. this instruments can distinguish the different changes of QOL of different people(healthy people and COPDs patients), different time(before and after treatment) and different curative effect of different intervening measures. It Even can distinguish the difference and degree of transverse data of different groups. this instruments is sensitive to all changes of score of QOL. So its responsibility is good.3.Analyzed results of validity and reliability indicated this instruments is good in its content validity and constructive validity. It has a moderate criterion-related validity. The relativities of domains are very well. At the same time, this instruments is good in split-half reliability and Cronbach's alpha coefficient. It indicates this instruments has a good validity and reliability.
