Object:1. To investigate clinical signs and symptoms in patients with lumbar disc prolapse is inconsistent with CT manifestations of proportion.2. To approach the correlations between the severity of the symptoms and the CT signals of nucleus pulposus herniation(NPH)of the lumbar disc protrusion sufferers and to find the closed CT signals to the severity of the symptoms of the lumbarl disc protrusion sufferers.Method:Only the patients who accord the inclusion criteria to be enrolled to the clinical observation, and all the 260 inpatients or outpatients come from Sichuan province Chinese medicine hospital from March 2014 to March February 2015.Collect each patients'CT indexes.Based on the improved evaluation scale (according to the Japanese Orthopaedic Association scoring system), all the cases can be divided into light group, moderate group, heavy group.Get the unmatched proportion,by comparing the dfferences between symptoms and signs with CT scaning.Adopting SPSS20.0 statistical software for data analysis. The distribution of the CT indexes and the symptoms and signs,is analyzed by adopt independent samples nonparametric tests. The symptoms and signs of CT evaluation index and lumbar intervertebral disc protrusion of correlation study, using multiple Logistic regression analysis to the classification of the multiple factors analysis, take 95% confidence interval, i.e., P< 0.05, with statistical significance.Result:1. Patients with symptoms and signs scores as mild, with a moderate but herniated 33 cases (36.67%), severe 19 cases (21.11%); Patients with symptoms and signs scores to moderate, but 14 cases of herniated mild (11.48%), severe 27 cases (22.13%); Score for severe patients with symptoms and signs, but 6 cases herniated mild (12.50%), moderate in 5 cases (10.42%). In addition, there are signs and CT in 4 patients showed herniated left and right are not consistent, accounted for 1.54%,2 cases with signs and CT shows that the level of herniated inconsistencies, accounting for 0.77%. Therefore, the study in the signs and symptoms of a total of 110 patients with CT manifestations, accounting for 42.31%.2. Nucleus pulposus in the cross section size, the outstanding position in the cross section of nucleus pulposus, nucleus pulposus compression of the dural sac, pathological type, density, the shape of the protrusion of nucleus pulposus CT evaluation indexes such as the signs and symptoms of lumbar intervertebral disc protrusion severity distribution has a significant meaning, P values were less than 0.05.3. Schmorl nodule patients symptoms and signs of lumbar intervertebral disc protrusion of severity distribution has no significance, P value is 0.126.4. By factor analysis, the impact on the severity of the patients with symptoms and signs have significant meaning of CT indexes including nucleus pulposus on the cross section of the outstanding size (including a domain, domain b. c domain), the outstanding position in the cross section (the nucleus pulposus prominent position on the outside). From mild symptoms and signs, compared with the signs and symptoms of severe, has the following features:On the cross section of nucleus pulposus domain, b, c domains highlight its compared in d field outstanding, make the risk in patients with symptoms and signs seerity of smaller (its risk degree OR values were 0.003,0.003,0.003); On the cross section of the lateral position of nucleus pulposus prominent compared with its location on the outside of a prominent, the risk of patients with symptoms and signs seerity bigger (OR= 315.748) (its risk degree). From the perspective of a moderate symptoms and signs, compared with the signs and symptoms of severe, has the following features:On the cross section of nucleus pulposus prominent to a domain, b, c domains compared with prominent to d domain, the signs and symptoms of patients with seerity of smaller risk (the risk degree OR values were 0.008,0.008, 0.008); On the cross section of the lateral position of nucleus pulposus prominent compared with in a lateral position, make the risk in patients with symptoms and signs seerity of big (OR= 93.770) (its risk degree).Conclusion:1. Approximately 42.31% of patients with clinical symptoms and signs is inconsistent with CT performance and the CT shows light signs and symptoms but heavy make up about 12.50%; CT shows heavy signs and symptoms but light accounts for about 21.11%.2. Nucleus pulposus in the cross section size, the outstanding position in the cross section of nucleus pulposus. nucleus pulposus compression of the dural sac, pathological type, density, the shape of the protrusion of nucleus pulposus CT evaluation indexes such as the signs and symptoms of lumbar intervertebral disc protrusion severity distribution has a significant meaning.3. The signs and symptoms of lumbar intervertebral disc protrusion severity significance of CT indexes including the influence of nucleus pulposus in the cross section of outstanding size (including a domain, domain b, c domain), the outstanding position in the cross section (the nucleus pulposus prominent position on the outside). From the point of cross section, on the cross section of nucleus pulposus prominent is smaller, the signs and symptoms may be more light; From the perspective in the cross section of nucleus pulposus prominent position, in the lateral position of nucleus pulposus prominent make the patients symptoms and signs may be more heavy.
