In 2007, the basic medical security for urban residents has been built this signs the comprehensive basic medical insurance system has been built completely. After several years, the basic medical insurance in our country has covered all the population of urban and rural areas. Now we need to focus on the external of this system. Namely, how can we to optimize the operating system and the internal structure of the basic medical insurance, in order to promote the basic medical insurance develop, and the core of this job is to reform the basic medical insurance management system. In our country, the division of the rural and urban medical insurance system has hider the long term development of system itself and depart the current economic situation. Therefor the right way and reasonable choice to solve this problem is to establish the management and handling system separately.Based on the survey in Yanan, Hanzhong and Xunyi, this article tries to explore its approach to reform and reform achievement. With the institutional change theory, the article tries to analyses the problem and experience of the basic insurance management system reformation in Shaanxi province. And after learned from some successful practices in developed countries and developing countries in the basic medical insurance management system. The article hopes to Provide suggestions for the reform of the basic medical insurance management system in Shaanxi Province. At last, the article believes that the key that the basic medical insurance management system reform is establish a unified management system and specify the relevant institution function.
