China has entered the aging society since the late 1990s,the number of disabled elderly has been higher than expected,due to the special nature of the elderly care,the traditional health care mode which depends on family has been out of date,because it is very hard to meet the needs of these disabled people.And institutional pension can provide professional medical care to meet the needs of the elderly personalized,therefore,we must analysis the situation of management and nursing homes,nursing homes have to solve the current supply and demand imbalance.This article is written through the questionnaire survey,field research,access to care institution staff and other investigation methods,analysis of the supply situation of old-age care institutions in Tianjin City,difficulties and problems in providing the service in the pension agency,is a system of admittance of pension institutions is not standardized,the second nursing homes do not have clear delineation of the target receives the elderly are the same category.And there is no difference in the service provision,cause the competitiveness is not strong,the high vacancy rate is high.Third,the bed service team has many problems,such as the number of professional nursing staff is so small,the training mechanism is not sound,the space of promotion for staff is not smooth,low degree of social recognition.Forth,there are great care risk for institution stuff to look after disabled elderly.The insurance is not sufficient to solve the nursing home elderly in receiving menace from the rear,leading to the degree of disability is more serious.Fifth,elderly institutional is not combined with health care and pension.Whether it is medical or nursing homes do two-way referral,was still in its infancy.At the same time according to the actual needs of the disabled elderly,put forward suggestions on Tianjin city pension agency services,clear government duty,strengthen the support of pension institution;through the transfer of pension insurance system the organization's risk;strengthen the propaganda and guidance to strengthen the construction of old-age nursing staf-f team;encourage the public to participate in the pension services;establish a mechanism of the third-party evaluation,reducing the occurrence of the dispute;vigorously develop "medical and old-age care fusion"geriatric nursing reform model.
