Depression is a common mental disorder with a high incidence,high risk and high recurrence rate.WHO pointed out that the lack of accurate assessment and medical staff are the two factors,leading to a low rate of patients receiving effective treatment.The Application of CD-CAT Technique in depression assessment,on the one hand can improve the accuracy of the diagnosis,pay attention to both macro and micro,analysis of depressive symptoms of the subjects,and can classify subjects accurately,which facilitates targeted treatment and intervention for different kinds of subjects.One the other hand,it can improve efficiency without losing any accuracy,and then reduce the burden of answering,save the doctors’ interview time.To a certain extent,this feature eases the situation of China’s medical resources.This study attempts to explore a new and effective diagnostic tests from the technical level to solve the current difficulties.Constructing a high quality item bank of assessing depression symptoms according to the diagnostic criteria of depression in ICD-10,and developing it into CD-CAT-D(Depression),the results show that:(1)According to the ICD-10 diagnostic requirement,a high-quality item bank of depression Symptoms Assessment was established based on the G-DINA model.Containing 136 items from 15 classic depression scales,measured 10 symptoms of ICD-10 in the assessment of depression,averaging 1.60 symptoms per item.All items were analyzed by item quality analysis(RMSEA <0.08,Disc> 0.3,no sex,age or area DIF).The symptoms of every item were evaluated by three experts,with an average Kappa coefficient of 0.693.Questionnaire A(70 questions)and B(87 questions)(23 items overlap)used to construct the item bank with high reliability and validity.The Cronbach’s Alpha and split-half reliability of Quesitionnaire A are 0.967 and 0.961,Questionnaire B is 0.975 and 0.970.They also with relatively high validity.The total score of questionnaire and SDS was moderate-high correlation,Quesitionnaire A is 0.899,B is 0.899,both are significant(p <0.01).(2)The developed CD-CAT-D for depression symptoms assessment is feasible,176 subjects were answered on the CD-CAT-D test.The test system shows good efficiency and validity.The numbers of items answered by subjects are between 13 to 40.Average per person answered 29 items.Comparing assessment results of CD-CAT-D with SDS and PHQ-9,Correlation coefficient are respectively 0.699 and 0.8,both are significant(p<0.01).Results with assessment of SDS and PHQ-9 are both correlated significantly(p <0.01).(3)CD-CAT-D can provide richer diagnostic information.The probabilities of depression symptoms appearing of subjects(symptom spectrum)are visualized in form of radar map.Depression patients(n = 30)showed the probability of 10 depressive symptoms were all above 0.5,and were higher than the probability of normal subjects(n = 146)on the corresponding symptoms.176 subjects were divided into 83 different kinds of symptom expression patterns.Among 30 patients with depression,"1111111111" pattern(10 depressive symptoms are all appearing)with the largest number,accounting for 36.67%.Among 146 normal subjects,"0000000000" pattern(subjects did not show none depressive symptoms is appearing)the largest number,accounting for 17.81%. |
