Effects Of Minimally Invasive Intracranial Hematoma Removal On Lactic Acid And CRP In Patients With Hypertensive Cerebral Hemorrhage

Objective To monitor the expression of arterial blood lactate and CRP in patients with hypertensive intracerebral hemorrhage,to monitor the effect of minimally invasive intracranial hematoma clearance on arterial blood lactate and CRP in patients with cerebral hemorrhage,and to explore the effect of minimally invasive surgery.Methods From October 2016 to October 2017,a total of 109 patients with hypertensive intracerebral hemorrhage(hypertensive intracerebral hemorrhage)received by emergency ICU in the affiliated Hospital of XX University were collected.Patients who met the criteria were randomly divided into two groups: minimally invasive intracerebral hematoma removal group(abbreviated as the treatment group)and the conservative medical treatment group(abbreviated as the control group).Among them,51 cases were treated with minimally invasive hematoma treatment group and 58 cases in the control group.The data included age,sex,volume of intracranial hematoma,Glasgow score on admission,time from onset to admission,lactic acid and CRP levels,underlying diseases and so on.Results 1.The blood lactate content in the conservative group and the minimally invasive group decreased significantly after operation or conservative treatment(P < 0.05),but there was no significant difference between the two groups on the second day after admission(P > 0.05),and on the third day of admission,there was no significant difference between the two groups.On the 7th day,the lactic acid of the minimally invasive group was significantly higher than that of the conservative group at the same time(P < 0.05),the 24 hCRP of the minimally invasive group and the conservative group was higher than that of the admission group(P < 0.05),the GCS7--10 score of the conservative group was higher than that of the minimally invasive group(GCS7--10),and the 24 hCRP score of the minimally invasive group was higher than that of the minimally invasive group(P < 0.05).The difference was statistically significant(P < 0.05).The postoperative mortality in the conservative treatment group and the minimally invasive group was statistically analyzed(P < 0.05).The rate of postoperative rebleeding in the conservative treatment group and the minimally invasive group was statistically analyzed(P < 0.05).Conclusion Arterial blood lactic acid increases in different degrees in patients with hypertensive intracerebral hemorrhage at the acute stage of hemorrhage.With the decrease of clinical intervention,minimally invasive intracerebral hematoma clearance can affect the decrease of blood lactate level in patients with intracerebral hemorrhage.Aseptic inflammation may occur in acute stage of intracerebral hemorrhage,and the level of CRP increases in varying degrees.Minimally invasive intracerebral hematoma clearance can alleviate this inflammatory reaction,and the mortality of patients with intracerebral hemorrhage treated by minimally invasive surgery is lower than that of conservative medical treatment.
