Objective: The main purpose of this study was to study the risk factors of PVT formation in patients with splenomegaly after liver cirrhosis,and to investigate the postoperative complications and PVT formation in patients with liver cirrhosis and splenospasm at high altitudes,and to analyze the possible factors of PVT formation.Make a prediction.Analyze the indicators of preoperative and postoperative examination in three hospitals with different altitudes to see if there are differences.Methods : Clinical records(2012.01-2017.10)of patients with liver cirrhosis and splenomegaly in three hospitals with different altitudes(Qinghai,Xi'an,and Wuwei)were collected to statistically analyze their general conditions and examinations,especially for the preoperative portal vein width.Surgical methods,spleen thickness(size),postoperative PVT,incision infection and pleural effusion,postoperative examination,and postoperative body temperature statistics,to see whether the thrombosis group and non-thrombosis group had statistical differences,and After analysis,the risk factors for the formation of PVT in patients were calculated and their boundaries were calculated.Patients from three hospitals with different altitudes were divided into three groups and their preoperative and postoperative indexes were compared to see if there were any differences.Results: Compared with the non-thrombosis group,postoperative PVT formation was statistically significant with the patient's portal vein width,elevation,and D-Dimer postoperatively,P<0.05.Patients in Xining area and Xi'an area had significant differences in portal vein size,living altitude,postoperative PLT measurement,postoperative PT,D-Dimer,postoperative body temperature,PVT formation rate,and surgical methods(P<0.05).Patients in Xining area and Wuwei area were compared with the diameter of portal vein,spleen thickness(spleen size),liver function child-page score,living altitude,postoperative PLT measurement,ALT,AST,PT,TT,APTT,D-Dimer,Postoperative body temperature,PVT formation rate,and surgical methods were significantly different(P<0.05).Conclusion: Two-factor multivariate Logistic regression analysis found that Logit P,inner diameter of portal vein,postoperative D-Dimer,and altitude have a certain effect on PVT formation after cirrhosis surgery.The comparison between the Xining patients and those in Xi'an in the plain area and Wuwei patients in the low altitude area found that there were no differences between the two groups in Xining patients before operation,fever,pleural effusion formation,and incision infection.There was some difference between the portal vein diameter and the spleen size compared with the plain and low altitude areas,and postoperative PLT,ALT,AST,PT,APTT,D-Dimer were all different.
