Risk Factors Of Acute Ischemic Stroke-associated Pneumonia In Xining Area And ISAN,A~2DS~2 Score Predictive Value Of The Research

Objective:To investigate the incidence and risk factors of acute ischemic stroke-associated pneumonia in Xining area,to evaluate the value of ISAN and A~2DS~2 scores in predicting the value of acute ischemic stroke-associated pneumonia in Xining area.Methods:To retrospectively analyze acute ischemic stroke patients who met the inclusion criteria in the Department of Neurology,People's Hospital of Qinghai Province(March 2016-September 2017)and investigated the incidence of acute ischemic stroke-associated pneumonia in Xining area;Factors were firstly analyzed by univariate analysis,and Logsitic regression analysis was used to obtain the risk factors for acute ischemic stroke-associated pneumonia.The ISAN and A~2DS~2 scoring methods were used to predict the value of stroke-associated pneumonia using the receiver operating characteristic curve.The area under the curve was compared to determine which score method had higher predictive value.Results:(1)A total of 512 patients were enrolled in the study,85 of whom had stroke-associated pneumonia(16.60%).(2)the risk factors were included in Logistic regression analysis.The risk factors for acute ischemic stroke-associated pneumonia in Xining area were male,aged?70,smoking,high homonymy.Cystine,atrial fibrillation,diabetes,dysphagia,disturbance of consciousness,NIHSS score>10points(P<0.05).(3)The area under curve of the ISAN score was 0.741(95%CI:0.701,0.779),Z=8.115,and the optimal cutoff value was 8 points.The area under curve for the A~2DS~2 score was 0.857(95%CI:0.823,0.886),Z=15.742,and the optimal cutoff was 4 points.The area under curve of A~2DS~2 score was greater than the ISAN score(Z=3.512,P<0.05),and the difference was statistically significant.Conclusions:(1)Meal,age?70 years old,smoking,homocysteine,atrial fibril lation,diabetes,dysphagia,disturbance of consciousness,and NIHSS score>10were risk factors for acute ischemic stroke-associated pneumonia in Xining are a;(2)ISAN and A~2DS~2 scores have good predictive value for acute ischemic st roke in Xining area area.A~2DS~2 score is more advantageous for predicting acut e ischemic stroke-related pneumonia in Xining area.
