Objective:To investigate the relationship between the red blood cell distribution width and the clinical and pathological features of IgA nephropathy patients.Methods:767 patients were enrolled who were diagnosed primary IgA nephropathy in Ning Xia Medical University General Hospital from Aug.2008 to Sept.2017,which were retrospectively analyzed.The data,which including demographic informations,laboratory examination and pathological data were collected.Then,the relationship between the RDW and the laboratory indexes and the pathological results was analyzed.Results:767 patients with IgAN diagnosed by renal biopsy with an average age of 35 years.There were 54.4%male patients and 45.6%female cases.(1)According to the interquartile method,the patients were divided into four groups(<12.10%?12.10%-12.50%?12.51%-13.10%??13.10%).When the red cell distribution width gradually increased accordingly,the levels of serum albumin were decreased accordingly,while the 24h urine protein,BMI and age gradually increased,mesangial cell proliferation degree showing a gradually serious trend.(P<0.05);(2)According to the e GFR with 45ml/min/1.73m~2 standard,patients were divided into two groups.Compared of group e GFR?45ml/min/1.73m~2,the patients with e GFR<45 ml/min/1.73m~2 group were older,with high levels of SBP,DBP,MAP,Scr,UA,24 hour urinary protein quantitative data,and RDW,lower levels of ALB and HGB(p<0.05).(3)The patients were grouped by 24 hour urinary protein quantitative data with 1g cutoff(24 hour urinary protein?1g group,<1g group).When compared with the patients in?1g group,the patients with 24 hour urinary protein<1g group had higher levels of BMI,DBP,MAP,Scr,UA,RDW,TG,and CHOL,lower levels of ALB and e GFR(p<0.05).(4)The patients were grouped according to the degree of the renal tubular atrophy/interstitial fibrosis.With the degree of pathological damage aggravated,contraction diastolic blood pressure,mean arterial pressure,uric acid,urine protein,24 hour urinary protein quantitation,red cell distribution width showing a growing trend,while the levels of ALB,HGB,and e GFR showed a decreasing trend(p<0.05).(5)Pearson correlation analysis showed that RDW was positively correlated with age and urine protein,and negatively correlated with HGB,MCV and ALB(p<0.05).Conclusions:RDW is associated with age,BMI,serum albumin,24 hour urinary protein quantitation,renal function and renal pathological damage in IgA nephropathy.The degree of the renal tubular atrophy/interstitial fibrosis is more prominent in IgA patients with elevated RDW.
