| objective: Lung cancer is the main malignant tumor that threats human’s health.As the most common lung cancer,the incidence of lung adenocarcinoma has far exceeded squamous cell carcinoma,so it has Received more and more clinical attention.Most commonly,the clinical course of the disease is insidious,Thus,early diagnosis rate of lung adenocarcinoma is low.Most patients were found at a late stage.With the development of medical technology and the deepening of understanding of lung adenocarcinoma,the way of treating adenocarcinoma of the lung had undergone an extensive antigenic change.Transit gradually from surgery、chemotherapy regimens and radiation therapy to Precise treatment of gene molecular level,particularly,the molecular targeting drugs produced by the EGFR gene mutation have been widely used in clinical treatment,and with excellent efficacy,Therefore,it is particularly important for the early prediction of the mutation of the EGFR gene in patients with lung adenocarcinoma.This study analyzed the Demographic,imaging features,and the correlation between the tumor markers and the mutation of the EGFR gene in the patients with lung adenocarcinoma.To accomplish Prediction of EGFR gene mutation,Guidance of EGFR-TKI drug therapy and provides a theoretical basis for the formulation of standardized molecular targeted treatment.Method: Retrospective analysis of clinicopathological features were used,Data were collected by reviewing medical charts of lung adenocarcinoma patients from March 1,2013 to July 31,2017 in the General Hospital of Nningxia Medical University.The collection of case information includes general conditions: age、gender、tobacco smoking history、family history 、pleural effusion、ethnicity、The level of differentiation、clinical stage、lymph node metastasis;imaging characteristics:including Lesion morphology、Edge morphology、Tumor and lung interface、internal structure、Adjacent structure、lesion size;The results of tumor markers CEA and CA199 as well as EGFR gene mutation type,EGFR gene mutation as the experimental group,EGFR gene were not mutated is the control group,by Retrospective comparative analysing the general conditions、CT imaging signs and the Correlation between tumor markers and mutation of EGFR gene of the test and the control groups。Result: A total of 157 patients were screened out,The mutation rate of EGFR gene was 51.6%,The components of exons 18,19,20 and 21 were 2.47%,46.91%,11.11% and 39.51%,There is no correlation between age,family history,pleural effusion and EGFR mutation(P>0.05),Sex,history of smoking,nationality,degree of differentiation,clinical stage and lymph node metastasis were related.It was considered statistically significant,The EGFR gene for women and non-smoking patients is more likely to be mutated,the rate of mutation was 51.55% and 64.89%,respectively.The mutation rate of EGFR gene in lung adenocarcinoma with high differentiation,stage I,stage II and no lymph node metastasis was significantly higher than that of low differentiation,stage III,IV and lymph node metastasis,the differences were statistically significant.The internal structure of CT imaging features is vacuolated and bronchoalveolar sign,which is related to EGFR gene mutation(P<0.05),The other imaging signs were not related to the mutation of EGFR gene(P>0.05),and there is A positive correlation between the tumor marker CEA and the EGFR mutation.Conclusion: The general conditions,imaging features,and tumor markers of the patients with adenocarcinoma of the lung can predict EGFR gene mutation and guide the therapy of EGFR-TKI drug and It provides a theoretical basis for the formulation of standardized molecular targeted therapy. |
