ObjectiveTo investigate the detection status of carotid atherosclerosis(CAS)in high-risk groups of stroke in a certain area of Ningxia,and analyze the related risk factors of CAS.MethodsAccording to the requirements“Technical Plan for Screening and Intervention of Stroke High-risk Population of 2015”,to select 4302 people of the more-than-40-year old(including 40 years)inhabitant from Shengli Township and Limin Community by cluster sampling method.Questionnaire surveys,physical examinations,laboratory tests,et al were conducted from January to March in 2016,Carotid ultrasound was performed in high-risk stroke populatiaon(a total of 704 cases)and to understand the detection status of CAS.The population were divided into CAS group and non-CAS group according to carotid ultrasound results.Basic information,disease history,personal lifestyle,physical examination,and test results were collected from two groups.All the data were processed and analyzed by SPSS 22.0statistical software.The age,waist circumference,neck circumference,and blood lipids in both groups were analyzed by t-test.Gender,nationality,stroke history,and hypertension,diabetes and other count data were analyzed by?~2 test,and logistic regression model was used to analyze the risk factors for CAS of high-risk groups of stroke.Results(1)A total of 796 cases(18.50%)with high stroke risk were screened out among 4302 screeners,and a total of 704 cases were carried out with carotid artery color ultrasonography,with an average of age of(59.34±9.12)years old,mainly aged60-69,accounting for 41.76%,the gender ratio of male and female was 1.54:1,and the prevalence of hypertension was 68.18%,which was very high.(2)The CAS,intima-media thickening,plaque formation and stenosis ocurred in high-risk stroke groups were 54.40%,9.09%,51.14%,and 31.53%,respectively.In the intima-media thickening groups,the ratio of the left side to the right side was 1.1:1;624 carotid plaques were detected,and 52.72%of the carotid plaques occured in carotid sinuses;almost all the carotid stenosis were mild stenosis(95.95%).The incidence of the left and right carotid artery plaques was about 1.1:1,and the ratio of bilateral stenosis was approximately equal(left:right=1:1).(3)The cases with CAS of high-risk stroke population,whose proportion of stroke,hypertension,diabetes,and age,TC levels,LDL-C levels were higher than non-CAS group,the difference was statistically significant(P<0.05).However,there was no significant difference in nationality,gender,smoking history,drinking history,TG,HDL-C between the two groups(P>0.05).(4)Logistic regression analysis showed that age,diabetes,and LDL-C were independent risk factors for CAS in high-risk stroke population in Ningxia.ConclusionThe detection rate of CAS in high-risk stroke population in a certain area of Ningxia was high,most of the occurrence of CAS was bilateral,and carotid plaques were prone to occur in the sinus region.Carotid ultrasonography in screening high-risk stroke population is of great significance to understand the detection of CAS in the high-risk stroke population.
