Objective : To investigate the distribution of pathogens with adult community-acquired pneumonia(CAP)and the prevalence of drug resistance of common CAP bacteria in adult patients in Qingdao.Method:The clinic data of 185 hospitalized patients with adult CAP were retrospectively collected in department of respiratory medicine from February2015 to January 2016.Carry out the bacterial culture for patients' respiratory specimens(sputum and pleural effusion?blood),monitor the pathogenic drug resistance,detect atypical pathogens in patients with serum antibodies by serological method.Result: Among 185 cases,Etiology detection positive 89 cases(48.1%),bacteria test positive 67 cases(36.2%),atypical pathogens test positive 28 cases(15.1%),among them 6 cases of mixed infection(3.2%),mixed infection of bacteria and atypical pathogens.From the bacterial culture and serological detection of 185 cases,pneumoniae was the most common pathogens,positive rate was 13.0%(24 cases),then followed by pneumonia klebsiella bacillus 12.4%(23 cases),mycoplasma pneumoniae 8.1% 5.9%(15 cases),haemophilus influenzae(11 cases),pneumonia chlamydia 5.4% 2.2%(10 cases),escherichia coli(4 cases),eosinophilic lung legionella bacteria 1.6%(3 cases),staphylococcus aureus,1.1%(2 cases),1.1%(2 cases),pseudomonas aeruginosa,acinetobacter baumannii,0.5%(1 case).Streptococcus pneumoniae of erythromycin resistant rate is higher.Klebsiella pneumoniae for high beta lactam drugs resistant rate than other antimicrobials.Among the patients with basic diseases and whose CURB-65 score equal and over 2,the detection of Klebsiella pneumoniae was significantly higher than other patients.There was no significant effect on the rate of pathogens detection between the use and no use of antibiotics.Moreover,there were no significant differences in the detective rates of pathogens between different seasons and ages.Conclusion: Streptococcus pneumoniae infection was the most common of adult community acquired pneumonia in Qingdao area.Klebsiella pneumoniae and Mycoplasma pneumoniae are also common pathogens in this area.Klebsiellapneumoniae detection rate was higher for the patients with basic diseases and those whose CURB-65 equal and over 2.Streptococcus pneumoniae is highly resistant to macrolide antibiotics.And Klebsiella pneumoniae is not sensitive to some beta lactam antibiotics.Atypical pathogens have high resistance to macrolides.According to clinical observation,priority should be given to quinolones in the choice of drugs for patients with high possibility of infection with atypical pathogens.
