Objective: Clinical use of methicocyclic acid to treat chronic subdural hematoma after drilling and drainage of residual hematoma,observe the effect of methicyclic acid on the absorption of residual blood after chronic subdural hematoma,and through a long period of clinical follow-up observation to understand whether the use of methicocyclic acid can reduce the risk of chronic subdural hematoma recurrence.Methods : A total of 46 patients with chronic subdural hematoma in the Department of Neurosurgery,Affiliated Hospital Affiliated Hospital of Southwest Medical University,07 months ~2017 year 2016,were treated with drilling and drainage surgery.According to the random grouping principle,23 cases were treated with methicinic acid and 23 cases in the control group.The treatment group was intravenous drip of Tranexamic Acid Injection(1g,bid)on the day after the operation.After 1 weeks of vein administration,the oral administration of methicate(250mg,TID)was replaced for 3 weeks,which was given for 4 weeks.The control group did not use methicocyclic acid for 6 months.CT examination(AV1,AV2,AV3,AV4,AV5,V1,V2,V3,V4,V5)in the treatment group were recorded at third,first,third,eighth,and sixth months after the Administration(the control group was followed up with V1,V2,V3,V4,V5).Through the modified dadfield formula,a total of 5 times of CT head examination were performed to calculate the changes in the residual hematoma of the subdural after the operation and the recurrence of hematoma within 6 months.The number of cases.Results: The hematoma volume AV1,AV2,AV3,AV4 and AV5 at regular follow-up in the treatment group were 21.02 + 5.07,18.85 + 4.84,10.25 + 6.01,5.43 + 4.89 and 2.78 + 1.85,respectively.The hematoma volume V1,V2,V3,V4 and V5 in the control group were 20.25 + 6.03,19.01 + and V5 respectively.There was no significant difference in the residual blood volume of subdural hematoma in the treatment group and the control group at third days and first weeks after operation(P > 0.05).The AV3,AV4 and AV5 in the treatment group were less than those of the control group,V3,V4,V5(P < 0.05)and the reduction trend of hematoma in the same period of the treatment group was more obvious than that in the blank group.After 6 months of follow-up,no recurrent patients were found in the treatment group,and 6 cases recurred in the control group.The recurrence rate of the treatment group was significantly lower than that of the control group(P < 0.05).Conclusion: Tranexamic acid promotes the absorption of residual blood after chronic subdural hematoma.The use of tranexamic acid can reduce the risk of recurrence of chronic subdural hematoma.
