Objective:To detect the serum pepsinogen(PG)concentration in patients with different gastric mucosal lesions and healthy subjects,and to explore the relationship between pepsinogen and various gastric diseases as well as the clinical value of pepsinogen in screening gastric disease.Methods:The subjects were selected from March 2017 to December 2017 in the digestive department of Jiangxi People's Hospital and the medical examiners in the medical center of our hospital.According to the endoscopy and pathological results,the subjects were divided into non atrophic gastritis(NAG)group,gastric ulcer(GU)group,chronic atrophic gastritis(CAG)group,gastric cancer(GC)group and healthy control group.3 ml venous blood was collected in the morning,the serum was separated,PGI and PGII were detected by enzyme immunoassay,and the PGI/PGII was calculated in strict accordance with the instructions of the reagent.The statistical analysis was carried out by SPSS17.0 software.The measurement data were expressed by x s.The variance analysis was used in the group.The test of two independent samples was tested by t test,and the count data was 2 test.The difference of P<0.05 was statistically significant.Results:Compared with the healthy control group,the level of PG I and PG II in the NAG group increased(P>0.05),and there was no statistical significance.The serum PG I,PG II and PG I / PG II decreased(p<0.05)in the GU group compared with the healthy control group.In group CAG and group GC,PG I and PGR decreased significantly compared with healthy controls(p<0.05).The screening criteria for GC were PG I < 70 ug/ L and PG I / PG II < 3.The positive rate of GC group and CAG group was higher than that of the GU group,NAG group and the healthy control group,and the difference was statistically significant(P<0.05).The sensitivity and specificity of PG I < 70 ug/ L and PG I / PG II < 3 for screening GC were 84.3% and75.9%.There was no significant difference in serum PG concentration between Hp positive group and Hp negative group(P>0.05).Conclusion:1.Serum PG can be used for the screening of gastric diseases,serum PG I and PG II increase,and PG I / PG II decrease suggests gastric ulcer,and the decrease of serum PG I and PG I / PG II suggests atrophic gastritis and gastric cancer.2.The level of serum PG is closely related to the structure and function of gastric mucosa.When PG I < 70 g/ L,PG I / PG II < 3 as the boundary value,PG screening GC has high sensitivity and specificity.3.Hp infection does not affect the concentration of serum PG.
