Objective:To evaluate both endocrine and exocrine pancreatic function who had recovered from acute pancreatitis in a follow-up study.Methods:One hundred and forty-three patients were subjected to tests of pancreatic function after an attack of pancreatitis.The mean time after the event was 1.7±1.5 years.Pancreatic endocrine function was evaluated by fasting blood glucose(FBG)and OGTT test.Pancreatic exocrine function was evaluated by fecal elastase 1.Factors that could influence endocrine function were also investigated.Results:The 143 AP patients,85 were male(59.4%)and 58 were female(40.6%).69 cases(48.3%)are biliary pancreatitis,52 cases are hypertriglyceridemic pancreatitis(36.4%),12 cases are alcoholic pancreatitis(8.4%)and 10 cases are idiopathic pancreatitis(7%).Mild pancreatitis was 31 cases(21.7%),moderate severe pancreatitis were 58 cases(40.6%),and severe pancreatitis were 54 cases(37.8%).The age is 48.4 + 13.1 years.Hyperlipidemia patients were 51 cases(56%),and the smoking and drinking patients were 32 cases(22.4%).The interval follow-up was 1.7 + 1.5 years.The surgical operation patients such as ERCP and cholecystectomy were 8 cases(8.8%).During hospitalization,there were 98 cases of pancreatic endocrine dysfunction(68.5%),of which 15 cases(10.4%),33 cases(23%)and 50(34.9%)in mild,moderate and severe acute pancreatitis(34.9%).The results showed that the severity of acute pancreatitis affected the endocrine function of acute pancreatitis(P < 0.001).The endocrine dysfunction of biliary,hypertriglyceridemic,alcoholic,and idiopathic pancreatitis were respectively 37(25.8%),42(29.3%),10(7%)and 9(6%).The results showed that the different cause of acute pancreatitis affected the pancreatic endocrine function(P < 0.01),and biliary pancreatitis was more likely to occur the abnormal endocrine function.The results of pancreatic endocrine dysfunction during follow-up were similar to those during hospitalization.With prolongation of follow-up interval,the endocrine function of pancreas increased gradually(P < 0.09).Sex,age,smoking,drinking and history of hyperlipidemia had no effect on pancreatic endocrine function(P > 0.05).The abnormal exocrine function of the pancreas was 7,accounting for 13% of the total.The results were not statistically significant compared with the different causes and severity of the acute pancreatitis.Conclusions:1.The severity of AP has an effect on the endocrine function of the pancreas.2.The different causes of AP affect the endocrine function which is greatly influenced by biliary pancreatitis.3.After AP,the endocrine function of the pancreas is impaired and the risk of pancreatic endocrine dysfunction is increased as time goes on.4.AP risk factors such as smoking,drinking and the history of hyperlipidemia did not affect the endocrine function of the pancreas.5.The exocrine function of the pancreas is impaired after the onset of AP.
