| Endoscopic papillary balloon dilatation(EPBD)is a safe and effective method for the treatment of choledochelithiasis.Nowadays small Endoscopic sphincterotomy(sEST)combined with EPBD is widely used,but the previous studies about the appropriate size of the balloon and dilatation time are controversial.Aim: Our prospective study is to compare the efficiency of different balloon dilatation duration with different size of balloon on the post ERCP pancreatitis.Methods: Patients were divided into the endoscopic papillary large balloon dilatation(EPLBD)(diameter:1.5cm)and endoscopic papillary small balloon dilatation(EPSBD)(diameter:1.0cm)group according to the size of bile duct stones,then they were respectively randomly allocated to the long balloon dilatation time(LT)(3minutes)and short balloon dilatation time(ST)(1minute)group,All the subjects have small sphincterotomy in the process of removing stones,then the corresponding balloon were used to remove the bile duct stone by the result of allocation,the rate of post pancreatitis was analyzed at last.Result: 217 patients were selected,the details are as follows,L-LT group(n=60,3 min)and L-ST group(n=63,1 min),S-LT group(n=48,3 min)and S-ST group(n=46,1 min).we observed that pancreatitis was noted in 6(10.0 %)patients in L-LT group and 6(9.5 %)in the L-ST group,This incidence was not significantly different(10.0 % vs9.5 %;p=0.929).There were 3(6.3 %)patients suffered post ERCP pancreatitis in the S-LT group and also 3(7.1 %)patients in the S-ST group,there were not significantly different as well.Logistic regression analysis result also show that either the dilatation duration of small balloon or large balloon is not be independent risk factor for post ERCP pancreatitis.Conclusion: 1 minute balloon dilatation and 3minutes.balloon dilatation may not show significantly different effect on post ERCP pancreatitis whether EPLBD or EPSBD. |
