Objective:To compare the effect of heparin protamine double titration and traditional methods on postoperative bleeding in patients undergoing undergoing cardiopulmonary bypass(CPB).Methods: Ninety adult patients scheduled to elective open heart surgery under CPB were randomly divided into three groups.In the conventional group(group A,n=30),375u/kg heparin was administered,and the amount of protamine was 1:1.2 of the initial heparin dose.In the heparin titration group(group B,n=30),heparin was given according to heparin titration,and the amount of protamine antagonist was 1:1.2 of the initial heparin dose.Heparin and protamine double titration group(group C,n=30),heparin titration combined with protamine titration.The general data of the patients were recorded.The blood transfusionvolume,postoperative flow volume and related complications in 24 groups were compared between the three groups.Results: group B and group C heparin dosage is less than the group A(P <0.05).The protamine dosage in group C is less than that in group A and group B(P <0.05).The thoracic drainage volume at 24 hours after operation were higher in the group A than in the groupB and group C(P < 0.05).Conclusion: Compared with heparin and protamine double titration and individualized anticoagulation,the traditional anticoagulant method used in clinic can reduce the amount of bleeding after CPB.
