Objective:To investigate the related risk factors of primary hypertensive and lacunar Infarction patients with cognitive impairment.Methods:Selected 292 hospitalized patients diagnosed primary hypertension from October 2016 to June 2017 in our hospital.According to the diagnosis of LI,all patients were divided into 2 groups:186 cases of hypertensive with LI group,106 cases of non-LI group.All participants underwent a carotid ultrasound,transcranial Doppler,craniocerebral CT or MRI,ambulatory blood pressure,Mini-Mental State Examination(MMSE)and laboratory analyses for lipids and homocysteine,and meanwhile the medical history was collected.All cases were divided into the normal cognitive group and the cognitive impairment group according to the total MMSE score and related assessment results.Univariate analysis was used to show the significant differences(P<0.05)among the cognitive groups,and binary logistic regression analysis was used to investigate the related risk factors of the cognitive impairment.Results: Among the 292 patients,174 patients were with normal cognition,118 patients were with cognitive impairment.There were significant differences in the MMSE score between the two groups,the score of the hypertensive with LI group(25.55±3.86)was lower than the other group(27.28±2.66)(P<0.01).The prevalence of cognitive impairment in the LI group was(47.31%)higher than the non-LI group(28.30%),the difference in prevalence between the two groups was statistically significant(P<0.05).Binary logistic regression analysis showed that age,years of education,coronary heart disease,cervical vascular plaque,nocturnal systolic were the independent risk factors for the cognitive impairment patients with hypertension and LI.The risk for hypertensive and LI patients with carotid plaques led to cognitive impairment was 2.59 times the risk for people without carotid plaques.The risk for hypertensive and LI patients with coronary heart disease led to cognitive impairment was 2.87 times the risk for people without coronary heart disease.Conclusion:Age,the years ofeducation,coronary heart disease,cervical vascular plaque,nocturnal systolic were the key risk factors for the hypertensive patients with LI led to cognitive impairment.
