Effectiveness Of Rectangle Flap Of Kasai Operation In Infants With Biliary Atresia:A Retrospective Study

Objective:To explore the role of rectangular flap in the Kasai operation treatment of type III biliary atresia.Methods:From Jan.2010 and Jan.2018,60 children with type III biliary atresia were operated in our unit.Accroding to the surgical procedures,they were grouped into RF(-)group and RF(+)group.this study is to analyze liver function recovery by comparing the DBIL andγ-GT,analyze the incidences of cholangitis by chi-square test and analyze native liver survival rates by Kaplam-Meier analysis.Result:In 60BA children,32 patients were under single Roux-en-Y hepaticojejunostomy,and 28patients were under rectangle flap lying on the jejunal segmental of Roux-en-Y hepaticojejunostomy.There were no significant differences of patients,gender,age,liver function before surgery.No significant differences existed in the incidences of cholangitis between RF(-)group and RF(+)group[84.4%vs85.7%,(?~2=0.21 P=0.885)].No statistical difference of postoperative liver function improvement in 12 months,18 months,24months,30 months,36 months.The 12month postoperative native liver survival rate were72.7%and73.3%,respectively.The 18month postoperative native liver survival rate were66.7%and73.3%,respectively.The 24month postoperative native liver survival rate were60.6%and63.3%,respectively.The 30-month postoperative native liver survival rate were 57.6%and60.0%,respectively.The 36-month postoperative native liver survival rate were 48.5%and46.7%,respectively.Conclusion:RF(-)group and RF(+)group has no significant difference in reducing the incidence of cholangitis and increase of survival rate in type III biliary atresia post Kasai operation.
