A Study On The High Risk Factors And Diagnostic Methods Of Pathological Upgrading After Atypical Hyperplasia Of Endometrium

Objective: To explore the diagnostic value of hysteroscopic endometrial resection for atypical hyperplasia of endometrium.Methods: The clinical data of 98 patients who underwent hysterectomy from January 1,2005 to December 12,2016 in the First Affiliated Hospital of Xinjiang Medical University and diagnosed with endometrial atypical hyperplasia by hysteroscopy were retrospectively analyzed.According to the pathological findings after hysterectomy were divided into endometrial carcinoma group(51 cases)and atypical endometrial hyperplasia group(47cases);according to the preoperative were divided into hysteroscopic curettage group and hysteroscopic endometrial resection in groups,to improve the preoperative diagnosis of atypical endometrial hyperplasia diagnosis the rate of the way.Results:In 98 patients,52.04% were complicated with endometrial carcinoma,the single factor analysis results indicate: the body mass index BMI?28kg/ ?,endometrial thickness,or postmenopausal,irregular vaginal bleeding,endometrial blood flow signal is stronger for endometrial cancer risk factors(P < 0.05);Multi-factor analysis results indicate: the body mass index BMI?28kg/?(OR=1.18,95%CI1.72~1.18,P=0.021),Ultrasound indicated endometrial thickness(OR=0.492,95 % CI0.234~0.996,P=0.049);endometrial blood flow signal is stronger for endometrial atypical hyperplasia after upgraded to endometrial carcinoma was statistically significant,(OR = 0.164,95%CI 1.032 to 0.849,P = 0.031).Compared with hysteroscopy,hysteroscopic endometrial electrotomy can improve the diagnostic conformation rate of atypical hyperplasia of endometrium,P=0.045.Conclusion:The incidence of atypical endometrial hyperplasia in patients with endometrial cancer is high,especially for patients with risk of endometrial ultrasound showed abundant blood flow signal of uterine endometrial cancer increased significantly,but the concurrency for the early differentiated endometrioid carcinoma;of atypical endometrial hyperplasia patients before hysteroscopy endometrial resection can improve the diagnosis rate of.
