Objective : To investigate the changes of left atrial function at different time intervals after PCI using real-time three-dimensional echocardiography(RT-3DE)and speckle tracking techniques.Methods: Twenty-five patients with single-vessel coronary lesions were enrolled.(The results of coronary angiography showed that one of the three major coronary arteries had a major coronary artery diameter stenosis ?75% with stent implantation(PCI),while the other two main coronary arteries.No stenosis or stenosis ?50%.Echocardiography was performed before and 1 month and 3 months after operation.42 patients with suspected coronary heart disease and negative coronary angiography were selected as control group.Three-dimensional echocardiography obtained LAVmax,LAVmin,LAVpre,left atrial total emptying volume(LAVt),left atrial total ejection fraction(LAVt EF)Left atrial passive emptying volume(LAVp),left atrial passive emptying fraction(LAVp EF),left atrial active emptying volume(LAVa),left atrial active emptying fraction(LAVa EF),left atrial volume index(LAVI),use Spot-tracking technique was used to obtain the strain-time curve of the left atrial wall.Left ventricular systolic,diastolic,late diastolic left atrial wall,atrial septum and left atrial overall longitudinal strain values(Ss,Se,Sa)were measured and calculated..RESULTS: Compared with left atrial volume in coronary heart disease group and control group,coronary heart disease was significantly different from control group except LAVt EF in coronary heartdisease before PCI(P<0.05);LAVp,LAVp EF The preoperative PCI of coronary heart disease was lower than that of the control group,and the other index was higher than that of the control group.Compared with the control group,the coronary heart disease group had 3 months postoperatively and 1month postoperatively,except for LAVp and LAVa EF,which were significant differences in the above-mentioned significant indicators.There was statistical significance(P<0.05),of which LAVp EF at 3 months postoperatively was higher than that at 1 month postoperatively,and all other indicators were decreased.Compared with the control group,LA-Ss and LA-Se were preoperatively in the coronary heart disease group.There was a significant difference between LA-Sa and LA-Sa(P<0.05).There were differences between LA-Ss,LA-Se,and LA-Sa at different time intervals before and after PCI in coronary heart disease group,but the difference was not statistically significant.Significance: Left atrial septum(Ss,Sa)increased1 month after surgery compared with the preoperative group,and left atrial wall(Ss,Se)increased more than 3 months after operation in the coronary heart disease group.Statistical significance(P<0.05).Conclusion: Real-time three-dimensional ultrasound and speckle tracking techniques can detect early left atrial volume and strain abnormalities in coronary heart disease patients without myocardial infarction;coronary heart disease group had one month after surgery and three months after surgery compared with preoperative group.Changes,including changes in left atrial volume parameters and strain parameters at 3 months after surgery were significant.
