Study backgroudAs far as concerned,it is revealed that more and more respiratory diseases are related to immune dysfunction.Pneumonia is the commonest pediatric respiratory diseases.In the pathogenes which can cause pneumonia,adenovirus does not account for a great proportion,however,it is able to cause sever pneumonia and have sequela that is hard to treat.The severity of pneumonia is related to the degree of immune dysfunction.The aim of this study is to explore the changes of T lyumphocyte subsets and cytokines secretion in children with adenovirus(ADV)infection,and to investigate the relationship among ADV infection characteristics,prognosis,and individual immune response.Patientd and methods40 cases of children with ADV antigen positive who were diagnosed pneumonia in our hospital from June of 2017 to December of 2017.All 40 patients are enrolled into this study.They are divided into two groups according to the diagnostic criteria of sever pnuemonia.Compare the level of the 6 cytokines and T lymphocytesubsets at the time of the disease onset and recovery.Results1?The proportion of T lymphocyte subsetsCD3(59.42±6.05%).CD4(32.89±7.64%)?CD8(23.08±8.90%)andCD4/CD8(1.28±0.37)in children who were diagnosed sever pneumonia decrease obviously than children with common pneumonia:CD3(71.65±3.78%).CD4(38.96±4.53%)?CD8(26.12±4.55%)?CD4/CD8(1.73±0.41)(P<0.05)?2.The proportion of T lymphocyte subsetsCD3(59.42±6.05%)?CD4(32,89±7.64%)?CD8(23.08±8.90%)andCD4/CD8(1.28±0.37)in acute phase is lower than recovery phase of children with common pneumonia:CD3(69.43±3.86%)?CD4(34.75±5.03%)?CD8(28.04±5.74%)?CD4/CD8(1.69±0.39)(P<0.05)?3.The level of cytokines in children who were dignosed sever pneumonia:IL-10(25.06±8.005pg/ml)?IFN-y(28.273±12.54pg/ml)rise obviously than children with common pneumonia:IL-10(15.65±19.06pg/ml)?IFN-y(7.215±6.665pg/ml)(P<0.01),however,the level of IL-6(26.65±15.586pg/ml)is higher compared to another group:IL-6(15.565±12.443pg/ml)(P<0.05),and the level of IL-2 and IL-4 is meaningless?4?The level of cytokines in two periods of children who were dignosed sever pneumonia.The level of IL-10(25.06±8.005pg/ml)?IFN-y(28.273±12.54pg/ml)in acute phase increase obviously than recovery period:IL-10(19.715±12.055pg/ml)?IFN-y(9.082±7.435pg/ml)(P<0.01),the level of IL-6(26.65±15.586pg/ml)in acute phase is higher tan recovery period:IL-6(18.025±16.437pg/ml)(P<0.05)?The level of IL-2 and IL-4 is meaningless.ConclusionThe expression levels of T lyumphocyte subsets in severe pneumonia caused by adenovirus decrease obviously campared with the levels in common pneumonia,it reveals that sever adenovirus infectin could lead to the decrease of cellular immune function.After treatment,the levels of T lyumphocyte subsets increase and cellular immune function achieve a new balance.The level of IL-10?IFN-? in children with sever pneumonia caute phase rise obviously than recovery period and children with common pneumonia.As a result,these two cytokines paly an important role in sever adenovirus pneumonia and are helpful to dignose it ahead of time.The average level of IL-4 in children with sever pneumonia caute phase does not change obviously compared with recovery period and children with common pneumonia,it is meaningless.
