OBJECTIVE Acquired immune deficiency syndrome(AIDS)is a chronic infectious disease caused by Human immunodeficiency viruses(HIV). Besides constitutional symptoms,the patients often can develop an opportunistic infection.Many patients do not know they are infected with HIV until they develop an opportunistic infection.The lungs are the most frequent sites of those infections.In this study,we analyzed clinical characteristics of 8 HIV/AIDS patients who were first diagnosed as sever pneumonia,in order to further understand HIV/AIDS for avoiding misdiagnosis and missed diagnosis.METHODS A retrospective,2-year study was carried out to assess the basic condition and clinical characteristics of HIV / AIDS patients in the First Affiliated Hospital,Medical School of Zhejiang University.These patients were first diagnosed as Sever Pneumonia.We used CURB-65 and PORT scoring system to assess the severity of the diseases.We also analyzed the relationship between danger ranking,organ function failure and mortality.RESULTS A total of 8 cases were included.Fever,cough and expectoration are the most common clinical manifestation.According to CURB-65 scoring system,3 cases scored 2,while 2 scored more than 3.As assesed by the PORT scores,2 cases belonged to gradeâ…¤,3 belonged to gradeâ…£,and 3 belonged to gradeâ…¢.All of 8 patients were complicated with one or more organ function failure.2 patients(25%)died.Of the 8 cases,2(25%)were pulmonary tuberculosis,2(25%)were pneumocyst is carinii pneumonia(PCP),and 1(12.5%)were,hacterial pneumonia,Distribution of infiltrates,consolidation or nodules in the segment or pulmonary lobes changes from hilum of lumg werw appeared in 2 cases.Enlarged hilar and mediastina]lymph nodes were found in these cases.CONCLUSION:When pulmonary infection is complicated with the follow conditions,HIV/AIDS should be considered:(1)A healthy puber or middle -aged individual who has history of blood transfusion,drug abuse, impurity sexual intercourse,or blood selling.(2)Multisystem impairment,even complicated with type B or C hepatitis.(3)Opportunistic algogens such as pneumocystis carinii,fungus,or colon bacillus are detected.(4)The focus of infection is hard to localization,and the manifestation of imaging examination is atypical.In that case, examination of serum HIV antibody is necessary,and is helpful for early diagnosis and treatment of AIDS and preventing the spread of the diseases.
