BackgroundProlonged QTc interval is considered to be an important indication for drug-induced arrhythmia,torsade de pointes(TdP)and sudden cardiac death.At present,it is found that the use of antipsychotic drugs in schizophrenic patients may cause QTc interval prolongation and arrhythmia,especially for patients with short course of treatment,which may cause sudden death by TdP,but the relevant research content is less.This study focuses on the study of the effect of the atypical antipsychotic drug Risperidone on the QTc interval in the drug-na?ve first-episode schizophrenia(FES)patients.The study of the individuals selected for the young age FES patients to reduce the influence factors of age on the cardiovascular system.The drug-na?ve patients reduced the effects of other antipsychotic drugs or antidepressants on cardiac function.The reliability of the study is high.ObjectiveAim to study the effect of atypical antipsychotic drug Risperidone on QTc interval in Drug-naive FES patients.MethodsTo minimize confounding factors,in this observational cohort study,only inpatients with a CCMD-3 diagnosis of FES patients and never previously treated with anti-psychotics or other psychotropic were included.For those inpatient accept Risperidone and with other antipsychotics,we conducted a retrospective cohort study in a naturalistic setting,in which patients were antipsychotics-naive and FES with three ECG measurements over 30 days,at baseline before the beginning of the treatment,at the second time with the drugs treatment of about 2 weeks,and also the third with the drugstreatment of about 4 weeks.The QTc interval was extracted to explore the relationship between Risperidone and prolongation of QTc interval in FES patients.A paired sample t test was used to analyze the three groups of values,Chi square test was used to analyze the number of abnormal QTc interval prolongation,and repeated measures ANOVA were used to analyze the factors related to drug use therapy,age and gender.Results1.The results showed that QTc intervals were statistically significantly prorogated after about 2 weeks or 4 weeks of Risperidone treatments,compared with the baseline(p=0.043,p=0.002).2.We sub grouped the data to check the confounding factors on QTc intervals,there was no statistically significant between multi-drug therapy and mono-drug therapy(p=0.081).We sub grouped the data by female and male.There was statistically significant between them(p=0.038).It showed that women had an influence on QTc intervals,and female patients were more influence to QTc intervals prolongation.Divided into groups according to age: ?30 groups and < 30 groups,result shows between the two groups,they have statistical significance(p< 0.001),compare the two groups,the sample group of age?30 had a trend higher risk?ConclusionIn summary,our study provided a dynamic acute drug effects on the changes of QTc interval in a relatively homogeneous samples of drug-naive FES patients at the earliest stage of antipsychotic Risperidone use.Those results reinforce the importance of monitoring the risk factors and assessing QTc prolongation at the beginning and throughout Risperidone treatment.
