Objections: To explore whether plasma adiponectin,high molecular weight adi Ponectin and the ratio of plasma adiponectin and high molecular weight adiponectin were related to the occurrence of lung cancer in patients with lung cancer who could not be operated on.To determination of plasma adiponectin components during disease progression,the effects of treatment on adiponectin levels.Methods : From January 2017 to January 2018,patients with lung cancer who were diagnosed as unable to receive surgical treatment were included as case groups,and healthy adults in our hospital at the same time were included as control group.If the patient has a history of cancer,diabetes,coronary artery disease,heart failure,liver and kidney dysfunction or failure,metabolic syndrome should be excluded.Fasting blood was extracted before treatment and centrifuged and stored in-80 ?refrigerator.After the treatment(radiotherapy,chemotherapy,targeted therapy)was finished,and when the the progress of the disease,Patient's fasting venous blood was taken.Plasma total adi Ponectin and high molecular weight adi Ponectin(HMWs)were determined by enzyme linked immunosorbent assay(ELISA)enzyme linked immunosorbent assaysa double antibody sandwich method.Test according to the kit instructions.The clinical data,including height,weight,age,sex and history,were collected,and Ki-67,blood routine,fasting blood glucose,liver function,renal function and blood lipid level were detected.The evaluation of curative effect was carried out according to the criteria for WHO solid tumor.The evaluation included complete remission,partial remission,stable disease and progression of the disease.The data were analyzed by statistical software SPSS20.0.The quantitative data of normal distribution or approximate normal distribution were expressed as mean ±standard deviation.T test or ANOVAV test were used to compare the plasma adiponectin levels before and after treatment with paired t test.Linear correlation and linear regression analysis were used for correlation analysis.The variable P ? 0.05 in univariate analysis is included in multivariate analysis.The multivariate analysis adopts Logistic regression model or linear regression model.The counting data were expressed by the number of cases or percentage,and the difference was statistically significant by chi-square test(P < 0.05).Results: 51 patients with lung cancer and 46 healthy controls were included.There were 17 cases of adenocarcinoma,21 cases of squamous cell carcinoma,13 cases of small cell lung cancer.The median diameter of the tumor was 5.4cm(1.7-11.7cm).The plasma adiponectin level of lung cancer patients was 1.37 ±0.31 ug/ml and that of control group was 3.36 ±0.62ug/ml(P < 0.001).The plasma HMW adiponectin level of lung cancer patients was 0.86±0.17ug/ ml,and the plasma HMW adiponectin level of control group was 2.46 ±0.37ug/ml(P<0.001).The ratio of HMW/ total adiponectin in lung cancer group was 0.66,healthy control group was 0.75(P=0.003).Lung adenocarcinoma,lung squamous cell carcinoma,small cell lung cancer patients with total plasma adiponectin levels were 1.4± 0.31ug/ml,1.36±0.3ug/ml,1.33 ± 0.31ug/ml(P=0.86);plasma HMW adiponectin levels were 0.86 ±0.16ug/ml,0.86 ±0.2ug/ml,0.88 ± 0.15ug/ml(P=0.94);plasma HWM/ total adiponectin ratio were 0.64 ± 0.16,0.65 ± 0.17,0.69±0.17(P=0.69).The size of Primary tumor,the ex Pression of Ki-67,fasting blood glucose,blood lipid levels and plasma total adiponectin,adiponectin HMW,HMW/ total adiponectin than no correlation or no linear correlation.plasma total adiponectin levels before chemotherapy was 1.36 ± 0.36ug/ml,after chemotherapy was 2.01±0.25ug/ml(P<0.001);plasma HMW adiponectin level before chemotherapy was 0.86± 0.2ug/ml,1.75± 1.45ug/ml after chemotherapy before radiotherapy(P=0.023).Plasma total adiponectin level was 1.42±0.40ug/ml,after radiothera Py was 1.93±0.25ug/ml(P=0.091);plasma HMW adiponectin level before radiotherapy was 0.99±0.23ug/ml,1.43±0.22ug/ml after radiotherapy and chemotherapy(P=0.184).Plasma total adiponectin level was 1.41±0.32ug/ml before chemotherapy,2.03±0.35ug/ml after chemotherapy(P=0.01);the plasma HMW adiponectin level of was 0.84 ±0.21ug/ml before chemotherapy,1.36±0.2ug/ml after chemotherapy(P=0.002).A total of 2 Patients underwent targeted therapy,the average total plasma adiponectin levels is 0.78ug/ml before targeted therapy,2.31ug/ml after therapy(P=0.106).Targeted treatment The plasma HWM adiponectin levels is 0.76ug/ml before targeted therapy,1.64ug/ml after treatment(P=0.064).Conclusions:(1)Plasma total adiponectin and high molecular weight adiponectin in Patients with lung cancer were lower than those in healthy controls.There was a difference in total adiponectin ratio between the two groups.(2)The plasma levels of total adiponectin and high molecular weight adiponectin in lung cancer patients after chemotherapy were higher than diagnose those in the treatment group.However,plasma total adiponectin and high molecular weight adiponectin levels did not change significantly after radiotherapy alone and targeted therapy,which may be related to the progression of lung cancer in only 2 patients in this study.(3)Weight loss,distant metastasis,tumor size,blood lipid level,fasting blood glucose did not affect plasma total adiponectin,high molecular weight adiponectin level and HMW/ total adiponectin ratio in patients with lung cancer.In conclusion,plasma total adiponectin and plasma HMW adiponectin may be protective factors for lung cancer,and lower adiponectin levels may be associated with the development of lung cancer.Early detection of adiponectin levels may be helpful for early diagnosis of lung cancer.Drugs targeting adiponectin pathway will become a new therapy for lung cancer.
