Diagnosis And Treatment Value Of Precancerous Lesions Of Duodenal Papillary Carcinoma In Endoscope

BackgroundDuodenal papillary neoplasms are rare in clinical practice and the prevalence rate is0.04-0.64%^[1].Late onset conceals its,early symptoms are not typical,can infiltrate into the pancreatic head tumor,easy cause bile duct of the pancreatic duct outflow obstruction,recurrent pancreatitis,with atypical hyperplasia and adenoma evil change rate is higher^[2].In recent years,with the popularity of endoscopy,the detection rate of duodenal papillary tumor is on the rise.Once the disease is diagnosed,the key to improve the survival rate is to choose a reasonable operation.This paper retrospectively analyzed the clinical data of 47 patients with duodenal papillary malignant tumor,precancerous lesion and benign tumor,aiming to explore the characteristics and diagnostic value of the precancerous lesions of duodenal papillary carcinoma in endoscope.ObjectiveStudy the Endoscopic characteristics of duodenal papilla precancerous lesions,improve the value of endoscopy in the diagnosis of duodenal papilla precancerous lesions,at the same time explore Endoscopic resection of the duodenal papilla?Endoscopic papillectomy,EP?the safety of the treatment and prognosis.Methods1.According to the pathological type,the patients were divided into three groups:duodenal papillary malignant tumor,precancerous lesion and benign tumor.The characteristics of the nipples?shape,mucosal surface,texture,brittleness,etc.?were compared between the three groups,and the characteristics of the endoscope under the precancerous lesions of duodenal papillary carcinoma were discussed.At the same time,the important value of endoscopic diagnosis was realized by comparing the diagnostic rate of imaging and endoscopic diagnosis.2.In patients with duodenal papilla precancerous lesion,due to personal choice treatment is different,divided into two kinds of EP,conservative treatment,by comparing the complications and the recurrence rate index,and then discusses the safety of the EP and prognosis.Results1.In patients with precancerous lesions of duodenal papillary carcinoma,the papillary hyperemia and edema and nodular changes of the nipple can be seen.2.Ultrasound,CT and MRCP found that the positive rate of duodenal papilla sites there are statistically significant differences??2=30.134,P<0.05?,of which the MRCP found nipple placeholder positive rate,such as the highest?46.2%?,followed by CT?25.0%?,the lowest for ultrasound?0%?.3.The diagnosis rate was significantly higher than the imaging detection rate.4.EP was used to treat the precancerous lesions of duodenal papillary carcinoma,with few complications and low recurrence rate.Conclusion1.The diagnosis of precancerous lesions of duodenal papillary carcinoma is of great significance.2.EP was used to treat the precancerous lesions of duodenal papillary carcinoma with high safety,low complication rate and low recurrence rate..
