| ObjectiveTo investigate the risk factors of cognitive impairment in Post-intensive care syndrome Patient(PICS-CI).To provide scientific basis for the prevention and reduction of cognitive impairment of patients with Intensive Care Unit(ICU)syndrome.MethodsThe patient who transferred from Post-intensive care unit(ICU)to the general ward for more than 7 days,and with the age more than 18 years old in the Third rate Hospital of Jinzhou Medical University from May 2016 to November 2017 were enrolled.The gender,age,marital status,education,occupation,salary,economic income,smoking,alcohol drinking,Previous of history,mechanical ventilation,the length of ICU stay,sedative and delirium,and initial diagnosis were recorded.According to mini-mental state examination(MMSE),the patients were divided into cognitive impairment(CI)group and non-CI group.The univariate analysis was performed to identify the risk factors of PICS-CI,and variables with statistical difference were selected to do multivariate binary logistic regression analysis for the confirm-able independence risk factors.Results1.PICS Cognitive impairment occurs: A total of 290 cases were selected,196 cases of men,134 cases of women,Age 18~78 years old,Average(52.56±14.38)years old,110 of the 330 patientsdeveloped CI,and the incidence was 33.33%.MMSE shows: directive force,the faculty of memory,Attention and computing power,Memory capacity,the language ability is severely impaired of the 110 patients.2.Univariate analysis showed that the risk factors of PICS-CI include gender,age,education,financial situation,smoking,alcohol drinking,previous of history,mechanical ventilation,the length of ICU stay,sedative and delirium,and initial diagnosis were recorded of ICU were main predictors of PICS-CI patients.3.It was shown by multivariate binary logistic regression analysis that the patients’ age above 60 years old [odds ratio(OR)=1.231,95%confidence interval(95%CI)=1.166~1.300,P<0.001),mechanical ventilation(OR=4.493,95%CI=1.116~18.095,P=0.005),ICU stays(OR=2.875,95%CI=1.531~5.400,P=0.001);sedation(OR=5.927,95%CI=2.143~16.394,P=0.001),and delirium(OR=5.871,95%CI=2.252~15.308,P<0.001)were PICS-CI independence risk factors,but the sex were protect factors(OR=0.102,95%CI=0.024~0.440,P=0.002).Conclusions1.The study found that gender,age,cultural degree,economic income,smoking,drinking,past medical history,mechanical ventilation,ICU stadys,sedation use,delirium and into the ICU patients with diagnosis were related to PICS-CI.2.For older people more than 60,using mechanical ventilation and staying in the ICU,use sedatives.Delirium is an independent risk factor for patients with PICS-CI,Gender is a protective factor.3.To prevent the occurrence of PICS-CI,slow cognitive impairment to dementia,medical personnel should be found early,against adverse symptoms promptly take effective nursing measures,improve the ICU from the patient’s quality of life has a great significance. |
