Objective:This study is a cross-sectional study based on the Study of Danyang Epidemiology and its main objectives are as follows:First,it aims at using immune markers to assess hypertension through studying the correlation between complement C3,C4,immunoglobulins(IgA,IgG,IgM)and hypertension and it can further reveal the pathogenesis of hypertension in immunology and seek the immunological targets for the prevention and cure of hypertension.Second,by exploring whether there are differences in complements C3,C4,immunoglobulins(IgA,IgG,IgM)between different physiques of hypertension,we seek an objective basis for the individual control of hypertension in TCM and provide new strategies for prevention and treatment.Third,having studied the correlation between TCM constitution and hypertension and explored the main constitutional factors affecting hypertension,we can prevent the onset of hypertension through the determination and intervention of constitution.Methods:From June 3,2017 to June 29,2017,according to the corresponding inclusion and exclusion criteria,we conducted on-site investigations and collection of data from 1035 residents of 5 randomly selected station communities in Danyang,Jiangsu Province.We collected the participants' general data including personal basic information,past medical history and information on drug use,collection of height,weight,and immediate blood pressure,etc.We filled in TCM Constitution Questionnaire and classified TCM constitution according to TCM Constitution Classification and Judgment Standard,and detected biochemical markers(plasma glucose,serum creatinine,blood lipids,etc.)and five immune indexes(complement C3,C4,IgA,IgG,IgM).Finally,the results were based on statistical analysis performed on the collected data.Results:1.Complement C3 is an independent risk factor for hypertension.Serum C3 concentration is positively correlated with blood pressure.The higher C3 is,the higher blood pressure is.The correlation between IgG and blood pressure is close to significant(P=0.05).There is no significant correlation between complement C4,IgM,IgA,and blood pressure.2.The TCM Constitution distribution of 1,035 participants was 385 in normal constitution(37.20%),182 in yang deficiency(17.58%),161 in phlegm-dampness(15.56%),112 in yin deficiency(10.82%),61 in qi deficiency(5.89%),43 indamp-heat(4.15%),31 in qi stagnation(3.00%),31 in special constitutions(3.00%),and 29 in blood stasis(2.8%).3.The prevalence rates of hypertension with different constitutions was 60%in phlegm-dampness,56.25%in yin deficiency,42.60%in normal constitution,36.07%in qi deficiency,29.67%in yang deficiency,25.81%in qi stagnation,25.81%in special constitutions,25.58%of damp-heat,and 10.43%of blood stasis.4.In all the 430 hypertensive patients,211 of them were male(49.1%),the mean age of them was 57.37±7.76 years,the mean systolic blood pressure of them was 141.1±14.8 mmHg,the mean diastolic blood pressure of them was 91.4±8.3 mmHg,the mean BMI of them was 25.1±3.1kg/m2,and 241 of them were treated with antihypertensive drugs(56%).The distribution of the five main physical constitutions of hypertensive patients was as follows:164 people were normal constitution,97 people were phlegm-dampness,63 were yin deficiency,54 were yang deficiency,and 22 were qi deficiency.5.No significant correlation was found between different body constitution complement C3,C4 and IgG in hypertensive patients.Conclusions:1.Complement C3 is positively correlated with blood pressure,and C3 can be used as a risk assessment indicator of hypertension.No significant correlation has been found between complement C4,IgA,IgG,IgM and blood pressure.2.The differences of complement C3,C4,immunoglobulin IgA,IgG,IgM between different body weights are not significant.3.The main constitutional factors affecting hypertension are phlegm-dampness and yin deficiency.
