Study The Efficacy Of Immune And Inflammatory Indicators In The Diagnosis And Assessment Of Idiopathic Membranous Nephropathy

Objective: To study Complement three(C3),Complement four(C4),Immunoglobulin M(IgM),Immunoglobulin A(IgA),and immunoglobulin in the serum of IMN patients.Immunoglobulin G(IgG),Immunoglobulin G4(IgG4),IgG4 to IgG ratio(IgG4 / IgG),C-reactive protein(CRP),Tumor necrosis factor ? alpha,TNF-?),to analyze the relationship between immune,inflammatory indicators and IMN disease,in order to further explore the diagnostic and disease evaluation efficacy of immune and inflammatory indicators for IMN.Methods: A total of 162 IMN patients,61 non-IMN disease control group patients,and 30 health checkups who were admitted to our hospital from May 2014 to May 2019 were collected.Serum C3,C4,IgA,IgM,IgG,IgG4,and IgG4/ IgG,CRP,TNF-? were measured,and analyze the differences of serum complement,immune and inflammation indexes between disease group and healthy control group,IMN group and non-IMN disease control group,and establish Logistic regression model and distinguish the verification model Ability and calibration ability;at the same time,analyze the correlation between serum immune and inflammatory indicators and renal function indicators in the IMN group to evaluate the correlation with IMN conditions,and study the differences in immune inflammation indicators between IMN patients with different conditions according to 24 h UP,and Multiple linear regression analysis was performed on 24 h UP and eGFR to evaluate the efficacy of immune inflammation indicators in evaluating the condition.Results: 1.Serum SCr,Urea,CysC,Uric,eGFR,24 h UP,Alb,A / G,TC,TG,HDL,LDL,RBC,Hb,C3,C4,IgA,IgM,IgG,IgG4 / IgG in three There were statistical differences between the groups,and TNF-? and CRP have high diagnostic efficacy in the disease group,reaching 97.90% and 91.90% respectively.2.SCr,Urea,Alb,TC,HDL,C3,C4,IgA,IgM,IgG,IgG4 / IgG,and CRP are of great value in the diagnosis of IMN diseases.The established logistic regression model can reach 94.7% in the diagnosis of IMN.High sensitivity for IMN diagnosis.3.C3,C4,IgM,and IgG are correlated with disease status in patients with IMN.Serum IgG levels are independent indicators that affect 24 h UP.CRP,C3,C4,and IgM are independent indicators that affect eGFR.Conclusion: IMN patients' serum immune and inflammation indicators are helpful for the diagnosis of IMN patients.The combined detection of multiple indicators is expected to improve the diagnostic efficacy of IMN.Immune indicators are helpful to improve the disease assessment performance of IMN patients.
