| Background:Diabetic kidney disease and Osteoporosis have become common and frequently-occurring diseases in China.For patients with DKD and OP,the metabolic imbalance caused by renal injury will further affect bone metabolism and aggravate the existing osteoporosis.In Chinese medicine,both Diabetic kidney disease and Osteoporosis are related to"kidney",so "kidney main bone" can be used as a starting point to explore their correlations and treatment methods.Objective:To explore the clinical effects of Yishen Jiangu Prescription in the treatment of Type 2 diabetes mellitus combined with early Diabetic kidney disease and Osteoporosis whose TCM syndrome were kidney deficiency and blood stasis,evaluating the efficiency and safety of this prescription.Method:60 patients with T2DM combined with early DKD and OP were enrolled in our study,all of whose TCM syndrome were kidney deficiency and blood stasis.Randomly divided into control group and treatment group with 30 cases respectively,the former was treated with western medicine(including controlling blood glucose,blood lipids,reducing urinary microalbumin,and bone health supplements)while the latter was combined with Yishen Jiangu Prescription on the basis of western medicine.Both of this two groups were treated for 3 months.Clinical symptoms,BMD and bone metabolism related laboratory tests including type I pro-collagen amino terminal propeptide(PINP),type I collagen cross-linked carboxy terminal peptide(s-CTx),blood calcium?urine calcium,blood phosphorus,urine phosphorus,parathyroid hormone(PTH),25-hydroxyl Vitamin D(25-(OH)D)were mainly evaluated as well as the effects of the prescription on kidney damage including ACR.Results:(1)PINP and urinary calcium were significantly improved in these two groups after treatment(P<0.01(treatment group),P<0.05(control group));so were s-CTx(P<0.05(treatment group),P<0.01(control group))and 25-(OH)D(P<0.01for all).There is no significant difference existed between the two groups after treatment in blood calcium,blood phosphorus,urinary phosphorus and PTH(P>0.05).After treatment,there is no significant difference existed between treatment group and control group except for PINP(P<0.05),urinary calcium(P<0.05)and 25(OH)D(P<0.01).(2)There was no significant improvement in neck of femur and lumbar bone density between the two groups after treatment(P>0.05).(3)ACR were decreased significantly after treatment in these two groups(P<0.01(treatment group),P<0.05(control group))and the treatment group decreased more significantly than the control group(P<0.05).(4)The improvement of clinical symptoms in the treatment group was better than that in the control group(P<0.01).The total effective rate in the treatment group was significantly better than that of the control group(83.30%vs.50.00%,P<0.01).Conclusions:(1)Yishen Jiangu Prescription can increase the level of PINP significantly,promoting bone formation by improving the activity of osteoblasts;it can significantly increase the level of 25-(OH)D and the efficiency of calcium absorption,improving osteoporosis;it can reduce calcium loss by reducing urinary calcium excretion;(2)Yishen Jiangu Prescription can significantly decrease ACR,reduce urinary albumin loss and improve kidney condition.(3)Yishen Jiangu Prescription has a good comprehensive effect on T2DM combined with early DKD and OP in patients whose TCM syndrome were kidney deficiency and blood stasis,and can significantly improve the clinical symptoms of patients.(4)The clinical trial lasted for 3 months.No obvious adverse reactions occurred during the trial and the safety was well. |
