Purpose:To explore the relationship between contrast enhanced ultrasound grade of carotid artery mixed plaques and acute cerebral infarction,Color Doppler ultrasonography and contrast-enhanced ultrasonography were used to investigated..Material and Methods:62 patients with acute cerebral infarction and mixed plaques of carotid artery were included in the present study,40 males,22 females,age 47-75(66.85±12.58)years old.62 patients underwent the examination of carotid artery contrast enhanced ultrasound and skull computer tomography(CT)or magnetic resonance(MR).The following clinical information were recorded:age,gender,blood pressure,blood sugar,history of smoking,total cholesterol level,high-density lipoprotein and low-density lipoprotein,etc.According to previous studies,plaques were divided into different types:hypoechoic plaques,mixed plaques(both have hypoechoic and hyperechoic),hyperechoic plaques.All the patients were diagnosed?as carotid atherosclerosis with mixed plaques,and the target plaques were mixed plaques of side of cerebral infarction whose thickness more than 2mm,no obvious hemodynamic stenosis.GE Logiq E9 ultrasound machine and equipped ML6-15 probe were used.The patient was told to breathe calmly,and then click "contrast" to enter the ultrasound contrast mode.Meanwhile,the bolus injected 1.2ml SonoVue contrast agent,and promptly injected 5ml 0.9%sodium chloride injection into the end note,and timed and stored the dynamic video recording.According to previous studies,classified contrast enhanced ultrasound plaques:?,plaques do not have enhancement,?,plaques have some point of enhancement inside or around,?,plaques have some point or line of enhancement inside or around,?,plaques have some point or line of pervade enhancement inside or around.Calculating cerebral infarction volume according to the results of CT/MR and references,divided patients into massive cerebral infarction group(?10 cm3)and not massive cerebral infarction group(<10 cm3).In this paper,two parameters of the grade of plaques' contrast enhanced and volume of cerebral infarction were analyzedResults:Of all the 62 patients,24 of them were massive cerebral infarction,38 of them were not massive cerebral infarction.Age,gender,proportion of hypertension,diabetes,coronary heart disease,smoking,dyslipidemia,and length of plague,thickness did not have any significance between massive cerebral infarction and not massive cerebral infarction(P>0.05).The carotid artery ultrasonography showed that:Compared with the patients with mixed plaques of enhanced grade ? and ? and patients with grade ? and ?,the volume of cerebral infarction in patients with grade? and ? was significantly increased.The cerebral infarction volumes of patients with mixed plaques and grade ? and ? were compared with those of patients with mixed plaques ? and ? respectively,and the differences were obvious(?P<0.001,?P<0.001).There was no statistically significant difference in the volume of cerebral infarction between the patients with grade ? and ? enhancement,and between the patients with grade ? and ? enhancement(P>0.05).Compared with not massive cerebral infarction group,the enhanced grade of carotid artery mixed plaques in massive cerebral infarction group were mostly graded as?(41.67%)and IV(33.33%),the enhanced grade of carotid artery convinced plaques in not massive cerebral infarction group were mostly graded as ?(8.33%)and ?(16.67%).The enhanced grade of carotid artery mixed plaques in massive cerebral infarction group was significantly increased,differences are statistically significant(P<0.01)..Conclusions:Contrast enhanced ultrasound were apt to obvious carotid artery mixed plaques new vessels' characteristics,thus to assess mixed plaques' vulnerability.Carotid artery mixed plaques classified higher by contrast enhanced ultrasound were more likely to occur massive cerebral infarction,mixed plaques contrast enhanced classification was closely related to volume of cerebral infarction and was one of the effective features of predicting cerebral infarction.
